Full Job Description

Since 1980, Tidewell Hospice has proudly served families across southwest Florida with compassionate, dignified care. As a member of Empath Health, we help patients with advanced illness live meaningfully while supporting their families with warmth, expertise, and respect.

Schedule: Monday-Friday, 8a-5p. Venice to Punta Gorda

Empath Health is seeking a RN Case Manager to join our South Facilities Team.

What you'll do:

The RN Case Manager beginning at admission the RNCM creates and maintains a plan of care, serving as the coordinator for patient and family interaction with Tidewell for the duration of the patient’s hospice experience. When a patient and loved ones think of Empath Health Hospice, they will think first of the RN Case Manager. The case manager must be able to listen, observe, communicate and sensitively provide nursing services in emotionally intense situations.
The RN Case Manager reviews and updates the care plan, physician orders, pharmacy profile and other records. The nurse also educates patients, family members and other caregivers about the disease process and decline, available palliative interventions, the dying process and safety practices.

What you'll need

Applicants must be a graduate of an approved nursing program and own a current Florida RN license. Minimum one year experience in an acute-care hospital environment providing direct patient care; previous experience as a hospice RN preferred.
