Full Job Description

We have a current opening for an Activities Coordinator for the Skilled Nursing Facility at our Portola Campus. This is a full-time position, five, eight-hour days per week. This position will require working days, and some nights.

Wage Scale: $19-$21

Position Summary:

The Activities Coordinator is responsible for developing, implementing and supervising the daily activity program for the SNF/LTC residents. She/he participates in the Minimum Data Set process with all the residents.

The Activity Coordinator reports to the Manager of the Skilled Nursing Facility and Resident Assessment Coordinator in their absence the Director of Nursing. The Activity Coordinator also coordinates activities with the Resident Assessment Coordinator.

Position Qualifications:

Must have two years experience in social or recreational program within the past five years, one year of which was full time in a resident activities program in a health care setting, or
Be an Occupational Therapist, Art Therapist, Music Therapist, Dance Therapist, Recreation Therapist, or Occupational Therapy Assistant, or
Have completed at least 104 hours of training in a course designed specifically for this position and approved by the State Department of Health.
Must be CPR certified.
Must be able to read and communicate effectively in English.
Completes annual education requirements.
Maintains regulatory requirements.
Must meet the physical demands of position, including vision, hearing, repetitive motion and environment as set standards require.
Demonstrates ability to remain calm in emergency and/or stressful situations and gives everyone the feeling of confidence that station is under control.
Exhibits genuine interest in and appreciation of adults in later years of life.
Must demonstrate a maturity level that enables him/her to perform the job in a professional and organized manner.
Must have a good understanding of asepsis to avoid and how infections among patient and self. Must have a good understanding of how to avoid contamination.

Primary Responsibilities:

Develops, implements and supervises the activity program.
Obtains from the Charge Nurse daily a report of those patients with changes in condition, appointments, and those unable to participate in activities that day.
Prepares activities assessment on all residents within 7 days of admission, evaluating each resident according to his/her background, interests, abilities, physical limitations, and needs for the purpose of planning meaningful programs.
Develops an activity program for each patient according to his/her individual needs.
Provides Nursing with a current calendar of planned activities.
Posts current activity calendar conspicuously for the information of the residents and staff.
Requests and maintains equipment and supplies.
Develops and maintains an appropriate schedule of activities for patients unable to leave their rooms.
Arranges and holds resident council meetings providing opportunity for resident involvement in planning and implementing the activity program.
Arranges religious programs.
Provides educational, social, exercise, and recreational activities both indoor and outdoor.
Arranges and conducts monthly outing for residents away from the facility.
When appropriate, trains and supervises volunteers to assist or augment the regular program.
Develops appropriate contacts with community agencies and organizations.
Participates in the quarterly patient care conferences with resident, his/her family and other facility staff.
Completes quarterly updates on all residents (more frequently if significant changes) and does quarterly notes & care plan revisions as needed.
Does daily charting on all residents, indicating daily activities, visitors, etc.
Attends in-service training programs related to the job.
Actively participates in performance improvement and continuous quality improvement activities.
Keeps in confidence any information learned concerning patients and hospital personnel.
Adheres to dress code, appearance is neat and clean. Reports to work on time and as scheduled, completes work within designated time. Wears identification while on duty.
Performs other duties as assigned.
