Full Job Description

Job Overview
We are seeking a dedicated and compassionate Case Manager or Registered Nurse Assessor to join our team. The ideal candidate will play a pivotal role in supporting older adults and caregivers facing various challenges affecting their quality of life. This position requires strong interpersonal skills and a commitment to helping clients navigate their circumstances while promoting their well-being and independence. The case manager provides needs assessments, case management, counseling and referrals, preliminary care planning and short term tracking of persons in need of long term support.

MAJOR JOB RESPONSIBILITIES:

1. Perform individualized comprehensive needs assessment to determine functional eligibility for various support programs.

2. Develop preliminary care planning.

3. Provide long term support counseling on topics and services affecting older adults, and caregivers, including public and private resource.

4. Make appropriate referrals to other agencies and/or programs for supports and assist with preparation of applications for publicly funded programs.

5. Conduct follow-up to assure the clients received needed information and/or services and are satisfied with the help received.

6. Provide advocacy on behalf of older adults and caregiver to assist in making their needs known.

7. Research and collect information on new public and private community resources for older adults and caregivers to ensure up-to-date resource files.

8. Stay current with federal, state and local legislation and policies that may impact older adults and caregivers.

9. Disseminate information to co-workers, clients, families, and community service providers.

10. Perform related work as required.

11. Attend all mandatory training as specified which may require some overnight travel.

12. Complete a minimum of 15 hours of continuing education training each year.

KNOWLEDGE, SKILLS, ABILITIES AND COMPETENCIES:

1. Working knowledge of community resources that assist older adults and caregivers, including referral procedures and eligibility criteria.

2. Excellent customer relations skills working with diverse populations, as evidence by the ability to communicate clearly and concisely both orally and in writing.

3. Ability to understand the policies, procedures and services provided by the State Department on Aging and other funding entities.

4. Knowledge of problem solving and conflict resolution principles, including active listening, interviewing and advocacy skills.

5. Ability to keep moderately complex records, to assemble and organize data and to prepare reports from such records.

6. Ability to deal with other employees, clients and other members of the public tactfully and courteously and to display cultural sensitivity.

7. Knowledge of office practices, procedures and equipment including skills in the use of personal computers and related software.

8. Maintain a valid driver’s license, good driving record, and access to a fully insured car.

9. Successfully pass a KBI background check.

This job description is not all-inclusive. The position may require other job-related responsibilities and job tasks other than those stated above.

QUALIFICATIONS:

Bachelor’s degree or a Bachelor's Degree with a Nursing License with preference to a degree in a human services field (social work, nursing, gerontology, psychology, sociology, etc.) from an accredited university, or a registered nurse licensed to practice in Kansas.
One year experience working in a human services or nursing environment with older adults.

Pay: $22.00 - $24.00 per hour
