JOB SUMMARY
The Weekend Transitional Care Specialist generates appropriate patient referrals from customers and promotes growth of referrals over time, with responsibility for coverage on Saturdays, Sundays, and designated holidays. Represents the organization's services and culture in a competent, professional manner consistent with interactions involving key physicians and hospital personnel. Explains the organization's services to patients and families in a manner that results in the admission of appropriate patients. Interacts with patients, families, and referral sources, and meets with physicians and hospital personnel during weekend coverage hours.
This position is ideal for a Registered Nurse (RN) who can bring clinical insight to weekend intake and referral conversations, but candidates with strong home health, hospice, or healthcare marketing/sales backgrounds are also encouraged to apply. Oversees growth and development targets, actively tracks progress, and maintains prospective market acceptance and allegiance during weekend hours. Works closely with the on-call clinical team in transitioning patients to services, and manages related expenditures in a fiscally responsible manner in accordance with the Company's budget.
KEY RESPONSIBILITIES
● Establish and maintain ongoing public relations efforts to increase community professional relationships and maintain relations with current and potential referral sources for the organization's programs.
● Provide dedicated weekend coverage for referral intake, ensuring no gap in service between Friday afternoon discharge planning and the following Monday.
● Obtain and record all data related to referrals received during weekend hours.
● Obtain and verify insurance coverage and financial status of potential admissions; provide complete and timely communications.
● If clinically licensed (RN/LPN), assist with basic clinical screening of referrals and communicate relevant clinical information to the on-call RN Case Manager; if not clinically licensed, effectively communicate with the on-call RN Case Manager and other team members regarding the status of potential admissions.
● Build and monitor working relationships as a high-quality provider of knowledge on community resources, customer perceptions, payer requirements, and patient/client views of Home Health services.
● Identify opportunities for additional or improved services to address unmet customer needs.
● Coordinate with the weekday Transitional Care team to ensure a smooth hand-off of pending referrals and follow-up items at the start and end of each weekend shift.
● Participate in staff meetings, department/sales meetings, team meetings, briefings, in-services, committees, and other related activities as needed, including occasional weekday meetings by arrangement.
● Assure compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
● Maintain regular attendance on all scheduled weekend shifts.
● Adhere to a code of conduct in accordance with Agency policy.
● Meet or exceed delivery of Company Service Standards in a consistent fashion.

QUALIFICATIONS
● Minimum age requirement of 18.
● Registered Nurse (RN) with a current, unencumbered state license strongly preferred; candidates with equivalent healthcare, home health, hospice, or marketing/sales backgrounds will also be considered.
● Ability to market and deal tactfully with customers, referral sources, and the community.
● Good communication, negotiation, and public relations skills.
● One or more years of marketing, sales, or clinical liaison experience (preferably in home health care or hospice) with a demonstrated history of positive results preferred.
● Valid driver's license and acceptable automobile standards and practices.
● Compliance with accepted professional standards and practices.
● Ability to work independently in an interdisciplinary setting, including during weekend hours with limited on-site supervisory support.
● Satisfactory criminal background check.
● Satisfactory references from employers and/or professional peers.
● Self-directed with the ability to work with little supervision.
● Flexible and cooperative with the ability to work in fulfilling all obligations.
● Reliable availability for Saturday and Sunday shifts, as well as designated weekend holidays, is required for this role.
● All new Transitional Care Specialists are subject to a 90-day probationary period.

SCHEDULE
This is a part-time, weekend-only position, non-exempt, covering Saturday and Sunday, generally 8:00 AM – 5:00 PM, with some flexibility required to accommodate referral source schedules, hospital discharge planning needs, patient/family meetings, and time-sensitive transitions. Coverage of designated weekend holidays may be required on a rotating basis. Occasional attendance at weekday meetings or in-services may be requested and will be scheduled in coordination with the employee. The role involves significant local travel by automobile within the assigned territory (mileage reimbursement provided in accordance with company policy).
Pay: $40.00 - $50.00 per hour
