Full Job Description

The Home Care Nurse is responsible for performing clinical assessments of new and current clients in order to develop thorough and accurate care plans in accordance with state regulations and company policies. Flexible hours allow you to work full time or part time.

Ensure care plans and caregiver medical forms are accurately maintained in CRM system and client files
Assign tasks and track adherence in CRM system so caregivers can report daily activity
Consults with client, family and Caregivers to provide a safe and efficient patient care and review progress
Conduct the clinical portion of new caregiver orientation to meet NJ Board of Nursing standards for CHHAs. Assess all newly hired CHHAs level of clinical competency.
Supervise and evaluate performance of CHHAs based on care provided to client and family.
Responsible for annual competency and performance evaluations of Home Health Aides to enhance the quality of care.
Reflect the core values of Home Instead at all times
Direct all personal care client services as per company scope of practice and provide overall coordination with office staff (Office Manager, Scheduler, Recruiter, Client Care Coordinator, Admin, etc)
Maintain a valid NJ RN license
Participate in staff meetings, track assessments and report on all pertinent issues in a timely manner

Qualifications include:

Current New Jersey Registered Nurse license in good standing by the NJ Board of Nursing
2 years’ experience in home care, hospice, visiting nurse or community health experience is preferred
Prior RN experience with in-home care assessments preferred
Excellent written and verbal communicator with strong interpersonal skills
Ability to work in a collaborative environment
Excellent computer skills
Must be self-motivated, organized, and able to work independently
Has a valid driver's license and a reliable car, willing to travel to clients' homes
