SMART® Integrated Behavioral Health Provider, LCSW/LICSW
SMART® Purpose: Our purpose is to break the cycle of poverty and positively impact the trajectory of lives by ensuring the wellness of children to support their educational attainment and competencies. We achieve this by proactively working with ALL students in a given school building to identify the physical, social, emotional, and behavioral barriers to classroom success. We then swiftly deliver interventions to address those barriers, thus creating optimal conditions for learning.
SMART® Clinics: Located and fully integrated within schools to proactively provide medical and behavioral health care services, our scope of care includes prevention and screenings; urgent care for minor illness and injuries; a CLIA-waived lab for testing including strep, flu, covid, urinary tract infections and hematocrit; chronic disease management; vaccinations; routine physicals; and individual, family, and group behavioral health services. SMART® Clinics are open in full alignment with school days and hours.

Position Summary: The SMART® Integrated Behavioral Health Provider provides care for the patient population by initiating and administering Primary, Secondary, and Tertiary tiered interventions, commencing with an integrative health assessment, which assists with risk and problem identification, then identifying and implementing appropriate interventions and follow-up strategies. Works in collaboration with the SMART® Team and student to define expected patient outcomes, describe appropriate resource utilization, and develop individual goal plans. Provides support/care coordination to the practice program to optimize patient outcomes while maximizing existing resources/partnerships within the school and minimizing duplication of services. Emphasis on supporting academic success in each plan of care and goal. Utilizes aggregate population data to ascertain the major population risk factors and/or social determinants and works collaboratively with the school partner and SMART® Team to identify and prioritize risks, as well as create and execute actionable solutions to mitigate the negative impact of those social determinants. Maximizes SMART®’s capacity to support a trauma-informed framework and healing, in keeping with the SMART® Whole Child, Whole School approach. Qualifications/Requirements:
· Master’s degree in Social Work from a program approved by the Council on Social Work Education.
· LICSW or LCSW
· Familiarity/Proficiency with motivational interviewing techniques, Penn Resiliency, Trauma Informed Care framework, and solutions-based care plan model types.
· Strong knowledge and experience with common pediatric and adolescent health problems.
· Experience working with pediatric patients and adolescents in a school or healthcare setting preferred.
· Ability to develop effective interpersonal relationships with the health care team, providers, and management team staff.
· Experience in providing managed care patient services.
· Commitment to work in community health with the challenges of a high risk, uninsured, and/or diverse patient population is helpful.
· Previous experience with EHR is helpful.
· Knowledge of HIPAA regulations and experience in information privacy/security laws, access, and release of information.
· Child Abuse Mandated Reporter Training is required.
· Experience working with computer systems; proficient in Microsoft Excel and Word.
· Ability to function independently, as well as work collaboratively with an integrative team.

· CPR certification required.
· Ability to successfully complete and pass background/Lifelinks.

Essential Duties and Responsibilities:
· Conduct integrated health assessments on all consented students to assess social, emotional, and other health related risks.
· Work closely with other members of the SMART® Team (i.e. FNP, RN/MA/CNA, Health and Wellness Coordinator, and Utilization Manager) to ensure both behavioral health and medical needs are addressed.
· Communicate efficiently and professionally with parents, teachers, and school administration, when appropriate.
· Plan, implement, and coordinate individual and group behavioral health interventions and/or therapy sessions to address individual needs of the student (i.e. anxiety, depression, anger, low self-esteem, bullying, grief, etc.).
· Conduct crisis management, when needed.
· Ensure referrals to higher levels of care and/or outside agencies are completed to facilitate the continuum of care.
· Assist in the development, revision, and implementation of patient programs to meet the specific needs of the school population as identified through health assessment and school leadership.
· Ensure notes/documentation are entered into eTHOS™, SMART®’s Knowledge Management System, and EHR in a timely manner.
· Maintain knowledge and comply with RHC regulatory and third-party payors’ procedures, notification requirements, and criteria, as well as the documentation required in medical records, etc.
· Create behavioral health interns’ daily schedule for all clinics, if applicable.
· Support the IBH team with screening of all incoming and outgoing DHR calls.
· Coordinate the facilitation of Anticipatory Teachings and Preventive Groups.
· Attend IEP meetings, truancy meetings, and any other meetings with the school, as needed.
· Be a liaison between GGC MSO, SMART® Clinics, DHR, and the Board of Education.
· Maintain security, confidentiality, and data integrity at both the individual and aggregate levels.
· Provide oversight to daily non-clinical SMART® operations, including scheduling, facilities issues, stakeholder communication, and troubleshooting.
· Report situations (e.g., escalated confrontations) to appropriate parties.
· Participate in SMART®/GGC MSO Team meetings and SMART® National Team meetings, as requested.
· Perform any other duties required by your clinic’s leadership and SMART®/GGC MSO.
Additional Key Daily Responsibilities
· Track referrals and follow-ups on patients by utilizing the eTHOS™ follow-up tool.
· Participate in daily huddle with the clinical team.
· Maintain close collaboration with school administration, faculty, behavioral health team, school social worker, school resource officers, and SMART® staff and providers.
· Complete all charting by the end of the day, including:
o EHR charting
o eTHOS™ charting

Confidentiality: The School District will execute a Business Associates Agreement (BAA) and a separate Non- Disclosure Agreement (NDA) that shall cover all patient health information (PHI), HIPAA, and FERPA.
Pay: $45.00 - $50.00 per hour
