Full Job Description

Resident Care Coordinator/Memory Care Director

As the RCC/MC Director at our Bellaview community, you will play a vital role in ensuring high-quality care and support for our residents. This position involves overseeing resident care plans, coordinating with healthcare providers, supporting caregiving staff, and maintaining compliance with state regulations. You will act as a key liaison between residents, families, and staff to ensure each resident receives the best possible care in a compassionate and respectful environment.

Location: Lehi, UT (On-site)

Job Type: Full-time

Schedule: Mon-Fri, 8AM-5PM, Weekends and On-Call as needed

Pay: $20.00 - $23.00 per hour

Responsibilities:

1. Resident Care & Oversight

Develop, update, and monitor individualized care plans.

2. Team Leadership & Coordination

Provide training, guidance, and support to caregivers.
Assist with staff scheduling and ensure proper staffing levels.
Maintain high standards of care through ongoing education and mentorship.

3. Communication & Collaboration

Serve as the main contact for residents and families regarding care needs and concerns.
Coordinate with healthcare providers (nurses, doctors, therapists) for optimal resident care.
Ensure residents' dietary needs and preferences are met during meals.

4. Compliance & Safety

Maintain accurate records of care, assessments, and incidents.
Ensure adherence to all state regulations and company policies.
Promote a clean, safe environment while following infection control and fall prevention procedures.
Assist residents with daily activities (ADLs) while promoting dignity and independence.
Support residents with medication reminders and health monitoring.

Requirements:

Active CNA license
Active CPR certification
1+ year of caregiving experience
Experience in senior care or assisted living settings (Preferred)
