$18-$22 per visit

Job Summary: We are seeking a compassionate and reliable CNA to provide high-quality, patient-centered care in a home health setting. In this role, you will support patients with daily living activities, promote safety, and work collaboratively with an interdisciplinary care team.

Essential Functions:

Assist with activities of daily living (ADLs), including bathing; grooming; oral hygiene; dressing; feeding and mobility assistance; transfers and ambulation support.
Follow and implement the patient’s plan of care under the direction of the Case Manager.
Provide skin care, nail care, foot care, and pressure relief to maintain patient comfort and safety.
Take and accurately document vital signs in the EMR; report abnormal findings to licensed staff.
Monitor and report changes in patient condition, including pain or safety concerns.
Ensure proper positioning and body alignment for patients.
Maintain a safe environment to reduce the risk of injury or health complications.
Educate and support patients with daily routines and care needs.
Collaborate with nurses, therapists, and other care team members.
Complete timely and accurate clinical documentation.

Requirements:

Professional Requirements:

Maintain HIPAA compliance and patient confidentiality at all times.
Adhere to all state, federal, and agency regulations, including supervisory visit requirements.
Follow infection control, safety, and quality assurance protocols.
Attend required trainings, in-services, and annual evaluations.
Maintain 12 hours of continuing education annually.

Qualifications:

Active CNA or Home Health Aide certification (state-approved program required).
Knowledge of basic nursing care and patient safety practices.
Strong communication and teamwork skills.
Ability to manage time effectively and maintain accurate documentation.
Basic computer skills and familiarity with EMR systems required.

Physical Demands

Prolonged standing and walking required.
Ability to lift, move and transfer patients weighing over 50 lbs.
Ability to identify patient needs, make quick decisions, and work under stressful conditions to meet deadlines.
Meet patient/family psychosocial needs.
Requires hand-eye coordination and manual dexterity.
Ability to utilize durable medical equipment in a home setting.
