Full Job Description

Job Overview
MetaMed is seeking a compassionate, organized, and patient-focused Healthcare Case Manager to join our growing team. In this role, you will coordinate and oversee patient care for individuals living with chronic conditions, ensuring seamless access to clinical services and an exceptional patient experience.

The Healthcare Case Manager serves as a key liaison between patients, healthcare providers, and interdisciplinary teams, helping patients navigate their care journey while promoting continuity of care, improved clinical outcomes, and high-quality service.

Key Responsibilities

Coordinate and monitor patient care plans to ensure continuity of care.
Follow up with patients throughout their treatment journey and provide ongoing support.
Manage referrals and coordinate services with physicians, specialists, and interdisciplinary healthcare teams.
Maintain regular communication with patients, providers, and caregivers.
Document patient interactions, care plans, and interventions accurately within the Electronic Medical Record (EMR) system.
Assist patients in navigating healthcare services and accessing appropriate community resources.
Promote quality patient care by identifying barriers and facilitating timely solutions.
Participate in patient education initiatives and provide guidance regarding treatment plans and available healthcare resources.
Ensure compliance with HIPAA regulations, organizational policies, and healthcare standards.
Perform additional duties as assigned to support operational excellence.

Qualifications

Bachelor's degree in Health Sciences or a related field.
Previous experience in healthcare case management, care coordination, patient navigation, or healthcare services preferred.
Experience working with patients living with chronic conditions is highly desirable.
Proficiency in Microsoft Office Suite and Electronic Medical Record (EMR) systems.
Excellent verbal and written communication skills.
Strong organizational, interpersonal, and time-management skills.
Compassionate, patient-centered approach with excellent customer service skills.
Ability to work independently while collaborating effectively within a multidisciplinary healthcare team.
Bilingual proficiency in English and Spanish (written and verbal) preferred.
