We are seeking an experienced Hospital Social Work leader with a strong background in acute care and case management leadership. This role is ideal for someone who brings clinical expertise, leadership experience and a passion for care coordination across the continuum.

To be successful in this role, you will bring:

A Master’s degree in Social Work and a Washington State LICSW
Experience leading teams in a hospital or acute care setting
A background in case management and care coordination
Several years of progressive leadership experience in healthcare
A strong clinical foundation with the ability to guide and support teams

Position Summary

The Manager, Social Work Case Management provides day to day leadership of a team of clinical professionals supporting hospital social work and cross continuum care coordination. This role partners with physicians, leadership and community partners to improve patient outcomes, enhance care coordination and support high quality, patient centered care.
