Full Job Description

Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families by assessing their needs, developing personalized care plans, and connecting them with appropriate resources. This position requires strong interpersonal skills and a commitment to improving the lives of those in our community.

Duties

Conduct assessment to determine patients needs pertaining to case management
Develop, implement, and monitor aftercare plans for patients in coordination with the clinical department
Provide guidance and support in navigating healthcare systems
Offer crisis intervention and management services to clients facing urgent situations.
Mentor clients by providing motivational interviewing techniques to encourage ongoing care post treatment
Collaborate with multidisciplinary teams to ensure comprehensive support for clients.
Maintain accurate documentation of client interactions
Stay informed about community resources and services available for clients, including addiction and mental health care
Submit any FMLA, Short term disability, or other related documents for patients while in treatment
Coordinate with clinical and medical team to obtain resources that fit patients needs

Skills

Proficiency in crisis management techniques and strategies.
Experience in behavioral health settings is required
Ability to effectively mentor clients through challenging situations.
Knowledge of motivational interviewing methods to facilitate client engagement.
Familiarity with child welfare policies and procedures is a plus.
Experience in addiction counseling or related fields is beneficial.
Excellent communication skills, both verbal and written, with the ability to build rapport with diverse populations.
Strong organizational skills with attention to detail in documentation and case management processes.

Pay: $23.00 - $25.00 per hour
