Job Duties:

Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the facility's administration and Nursing Policies and Procedures.

The primary purpose of your job position is to manage the day-to-day RN's, LPN's and Certified Nursing Assistants in accordance with current federal, state and local standards governing the facility, and may be directed the Director of Nurses Services or the designee, to ensure that the highest degree of quality care is maintained at all times.
Delegation of Authority:As a Supervisor you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned roles.
Direct the day-to-day function of the staff on the unit.
Contacts the residents physician and family to notify them of a change in the resident's condition or accident/incisent; also notifies the supervisor covering.
Periodically meet with assigned staff in planning services, programs and services for residents.
Admit, transfer and discharge residents as required.
Assumes all responsibilities of the RN Supervisor in his/her absence or as directed by the DNS/ADNS
Assist and participate in developing for each resident a preliminary and comprehensive assessment and written are plan that identifies the nursing and medical problem/and or needs identified.
Encourage resident and family to participate in the development of the plan of care.
Monitor residents to ensure that the plans are being followed and that the resident’s needs are being met.
Review nurses notes for the previous twenty-four hours for residents identified on the daily report.
Participate in orientation of new residents and family members. Ensure that room are prepared for the new resident.
Ensure all nursing services personnel are performing their respective duties in accordance with written policies and procedures.
Participate in a quality assurance studies are required.
Participate in surveys and inspections by government agencies.
Make periodic decubitus rounds to monitor the treatments and progress of wounds.
Supervise and instruct all non-licensed nursing personnel so policies are followed.
Assign duties to non-licensed nursing staff based upon the qualifications of the staff and the needs of the resident
Ensure the rehab programs are being carried as ordered.
Assumes independent responsibility for completing the required routine documentation and auditing as required.
Communicates and collaborates with the interdisciplinary team and or physicians.
Have the ability to assess, plan, implement and evaluate nursing care according to the individualized needs.
Guides the staff and ensures tasks are being completed.
Reports to the Director of Nursing

Qualifications:

Must have a valid NYS RN License
Previous experience in nursing home/rehab a plus
Current registration to practice as a Registered Professional Nurse
Reliable transportation and demonstrate excellent work ethic.
Must have excellent interpersonal, communication and organizational skills.
Must be able to lead and motivate the team.
Minimum of two (2) years of nursing experience and at least one (1) year of experience in a nursing leadership role
