Full Job Description

Position Summary

We are seeking a highly organized and proactive Care Coordinator to join our Homecare team. The Care Coordinator is responsible for managing patient cases, coordinating caregiver schedules, and ensuring all services are delivered efficiently and in compliance with agency and state regulations.

This role is essential in maintaining continuity of care, supporting both patients and caregivers, and ensuring that all authorized services are properly staffed and documented.

Key Responsibilities

Coordinate and schedule Home Health Aides (HHA) and Personal Care Aides (PCA) for patient cases
Ensure all patient cases are fully staffed according to authorized hours
Monitor daily schedules, call-outs, and replacements to avoid service gaps
Communicate with patients, caregivers, and family members regarding schedules and care updates
Track and update authorizations, care plans, and service hours
Work closely with HR and Recruitment to address staffing needs
Maintain accurate and up-to-date case documentation
Address and resolve service issues, complaints, or conflicts in a timely manner
Ensure compliance with agency policies and state regulations
Assist with audits and reporting as needed

Qualifications

Previous experience in homecare coordination or healthcare scheduling required
Knowledge of HHA/PCA services and homecare regulations
Strong organizational and multitasking skills
Excellent communication and problem-solving abilities
Ability to work in a fast-paced environment and handle high caseloads
Proficient in Microsoft Office and scheduling systems
High school diploma required; associate degree preferred

Ability to manage multiple cases and priorities simultaneously
Attention to detail and compliance tracking
Professional and team-oriented mindset
Bilingual (Spanish/English) is a plus
