JOB SUMMARY: The Case Acceptance Specialist plays a vital role in ensuring a seamless admission process for new patients in a licensed home health care agency. This position is responsible for evaluating, coordinating, and facilitating the acceptance of new patient cases, ensuring compliance with all regulatory guidelines, and maintaining high levels of communication and collaboration with referral sources, patients, families, and internal teams.
KEY RESPONSIBILITIES:
Case Acceptance Coordination:
Review and evaluate referral information for accuracy, completeness, and appropriateness for home health care services. (Case offers)
Verify eligibility for services based on payer sources, clinical needs, and agency capabilities.
Collaborate with clinical staff to assess patient needs and ensure the availability of appropriate services.
Ensure all documentation and authorizations are in place prior to accepting cases.
Communication and Collaboration:
Serve as the primary point of contact for referral sources, patients, and their families during the onboarding process.
Maintain clear and timely communication with internal teams, including the clinical, coordination, and billing departments.
Address questions or concerns from patients, families, or referral sources regarding the admission process. Seek assistance when necessary.
Conduct welcome calls upon receipt of member information and update operating systems accordingly.
Documentation and Compliance:
Ensure all required documentation pertaining to services like insurance authorizations, is obtained and completed accurately.
Enter patient data into the agency’s electronic health record (EHR) system promptly and accurately. When needed.
Maintain compliance with state, federal, and agency regulations related to patient admissions and care.
Ensuring Alerts and TALs are completed upon accepting members.
Ensure initial visit and notes are updated.
Operational Support:
Monitor the status of open cases and follow up with referral sources to expedite the intake process.
Escalate issues or barriers to case acceptance to the Intake Manager or Director of Patient Services as needed.
Monitor case acceptance logs and address discrepancies when they arise.
Identify opportunities to streamline the intake and case acceptance process for improved efficiency and patient satisfaction.
QUALIFICATIONS:
High school diploma or equivalent required; associate or bachelor’s degree in a related field preferred.
Minimum of 1-3 years of experience in home health care, case acceptance coordination, or case management.
Familiarity with home health care services.
Strong organizational skills with the ability to manage multiple tasks and deadlines.
Excellent communication and interpersonal skills to interact effectively with referral sources, patients, and team members.
Detail-oriented with strong analytical skills to review and process patient information accurately.
Microsoft Office Suite and Excel.
- Knowledge of state and federal regulations for home health care admissions.
Key Performance Indicators (KPIs):
Timely processing of new referrals and patient admissions.
Accuracy of completed documentation and data entry.
Satisfaction levels of referral sources, patients, and families during the intake process.
FUNCTIONAL ABILITIES:
Must be able to remain in a stationary position (e.g., seated or standing) for extended periods while performing desk-based tasks.
Must be able to operate a computer and other standard office equipment (e.g., copier, scanner, phone system).
Must be able to communicate effectively in English with internal and external contacts in both verbal and written form.
Must be able to read and understand in English written documents and data on a computer screen and in printed form (font size 12 or larger).
Must be able to exchange accurate information in one-on-one and group settings.
Must be able to occasionally lift or move office supplies and materials weighing up to 20 pounds.
Must be able to perform multiple tasks and manage frequent interruptions in a fast-paced office environment.
Must be able to travel within the office or between departments as necessary to complete tasks.
