Full Job Description

****UNDER NEW MANAGEMENT****

JOB TITLE:

Case Manager

You must be:

A certified peer support worker
Comprehensive community support services trained
A certified family peer support worker
License must be in good standing with the state of New Mexico, if applicable
Must be at least 18 years of age
Must have legal right to work in the United States
Must be able to speak, read, and write in English
Must be willing to participate in pre-employment screening and drug screen

Essential Function: Under general supervision, provides support services to clients who are receiving services through the agency. Experience in navigating supportive services like housing, job skills development, life management, and other social skills necessary for managing a life of recovery.

Core Competencies:

1. Needs of Person Served: Understanding of Substance Use Disorders. Recognizes the complex context of substance abuse. Understands co-occurring Mental Health disorders and impact on treatment.

2. Clinical Skills Needed for Position: Has a working knowledge of the treatment of Substance Abuse across varying models, theories, philosophies, practices, and treatment outcomes.

3. Person-centered Plan Development: Has a working knowledge of the principles of person-centered plan development and implements into practice.

4. Interviewing Skills: Ability to engage effectively in clinical interviewing, including diagnostic, service planning, and Motivational Interviewing techniques.

5. Evidence Based Practices: Knowledgeable regarding program approved EBPs

6. Clinical Risk Factors: Able to identify risk factors for suicide, violence and other risky behaviors in a supportive and respectful manner. Knowledgeable regarding interventions for safety when risk factors are identified to prevent or de-escalate a potential crisis scenario in the least restrictive methods.

Position Duties:

1. Effectively practices under the guidelines Certified Peer Support Workers/Comprehensive Community Support Services scope of practice

2. Supports persons served with vocational needs; resume building and interview skill building.

3. Understands and practices cultural diversity to include selecting appropriate strategies and resources for treatment.

4. Communicates regularly with counseling team on clients’ support needs, plans and progress.

5. Provides support with housing needs of the persons served.

6. Facilitates groups on topics within CPSW/CCSS scope of practice; housing needs, vocation skill, life skill building, etc.

7. Meets with clients to evaluate their support needs

8. Creates and tracks goals and tracks progress towards goals with clients

9. Document all interactions timely and accurately under the criteria of state licensing and CARF standards

10. Identifies care coordination needs and assists in linking clients with a care coordinator

11. Works with persons served and primary counselors on aftercare plans

12. Work cooperatively with other community resources and referral sources to coordinate services to clients.

13. Provides updates and progress reports to the court system, as needed.

14. Identifies the need for crisis intervention services and seeks immediate assistance from counseling staff

15. Maintains appropriate state license/registration/certification for scope of practice and participates in clinical supervision.

Organizational Function:

1. Carry out other responsibilities as delegated.

2. DEPENDABILITY-Employee can be counted on to complete assigned tasks in a timely manner with little supervision. Accepts responsibility when necessary to see that the job gets done.

3. ATTENDANCE AND PUNCTUALITY-Is prompt and on time for work, assignments and meetings. Notifies supervisor prior to lateness or absence. Understands that excess absences pose a hardship on other employees.

4. JOB SKILLS AND KNOWLEDGE-Employee completes all chart related documents and correspondence for assigned clients in a timely manner.

5. Employee provides individual counseling, case management, skills training, and facilitates educational, skill development and counseling groups as assigned and properly and promptly charts these activities in client records.

6. Employee works cooperatively with other community resources and referral sources to coordinate services to clients.

7. QUANTITY AND QUALITY OF WORK – Cares about the quality and accuracy of work being produced. Accepts responsibility for completing job tasks. Uses time productively and efficiently.

8. ATTITUDE – Demonstrates a cooperative, positive, and enthusiastic attitude toward fellow employees, clients and visitors. Maintains a positive attitude in carrying out assignments and is helpful and courteous to fellow employees.

9. CONFIDENTIALITY-Know and follows confidentiality law and procedures at all times.

10. COMMUNICATION SKILLS-Is effective in written and oral expression. Demonstrates proper telephone etiquette and is able to communicate easily with clients, referral sources, and visitors in an appropriate manner. Relates to clients and co-workers in a respectful and professional manner.

11. CONDUCT – Conducts oneself in a manner consistent with the agency’s code of ethics and code of conduct. Shows professionalism in the office setting.

12. FLEXIBILITY / HANDLING EMERGENCIES – Responds well to changing situations or routines. Handles crisis situations calmly and professionally.
