Full Job Description

Physicians transitioning from residency (3rd year) to practice are encouraged to apply.

Responsibilities

Function autonomously to care for patients with acute, critical and chronic health conditions.
Coordinate care management proactively working with the healthcare team and facility partners to produce good outcomes.
Provide medical oversight of patient care and rehabilitation.
Ensure proper progress on plan of care and identify any early warning signs for readmissions during the critical, immediate post-discharge period
Coordinate appointment for evaluation by specialist as needed for patients during their stay at the care facility
Work individually and in interdisciplinary teams of resources and consultants
Facilitates patient transition within and between health care organizations to ensure accurate communication among the health care providers
Record and maintain patient records in timely manner to ensure all services are accurately accounted for and available for review by the interdisciplinary teams
Timely documentation, discharge summaries, medicine reconciliation, etc. in EMR
Talk to patients about effectively managing their health, as well as assisting in designing appropriate treatment plans and recommending further care

Skills

Demonstrated problem-solving and workflow management
High level of clinical proficiency and excellent decision-making skills.
Ability to work independently and as an effective member of a healthcare team.
Excellent oral and written communication skills.
Ability to effectively collaborate and build relationships with patients, families and all care providers
Proven organizational skills and ability to prioritize effectively.
Strong computer skills with knowledge and experience with Electronic Medical Records

Qualifications

Current unrestricted licence MD/DO
Current and unrestricted DEA certificate

Job Type: Part-time

Pay: $200.00 - $300.00 per hour
