Full Job Description

Job Title: Housing Case Manager

Reports to: Permanent Supportive Housing Ops Director

Classification: Non-Exempt, Hourly,40 hours per week

SUMMARY

The role of the Housing Case Manager is to assist clients by facilitating and monitoring the on-going supportive service networks needed for individuals to achieve their maximum level of independence in HALO’s housing programs. This role also includes assigned HUD housing inspections, managing leases and communication with landlords, eviction prevention, and move in/out coordination.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

· Assist new clients with the move-in process (moving, providing bedding and furnishings, and household needs).

· Assess assigned clients and family needs which includes mainstream benefits.

· Develop a housing stability plan with each client entering the program, monitor the plan and provide needed support and resources for client to achieve goals and objectives for successful exit from a limited time frame program.

· Refer and link clients to appropriate resources (including mainstream) to address needs and help coordinate services as needed and perform follow-up activities.

· Attend meetings with client and other community service providers when doing so would be beneficial to the client’s success.

· Refer clients to employment assistance and encourage employment as they are able.

· If employment is not an option due to disability, refer client to ADRC and other resources to increase income.

· Hold regular case management meetings in the home, which may be weekly but must be monthly at a minimum.

· If case management meetings are held by phone or other means, must visit the home at least once

monthly to address any issues with the unit.

· Perform crisis intervention, and eviction prevention activities to assist the client with maintaining housing. This includes advocating with the landlord and addressing housing issues with clients.

· Fully explain leases to clients so they are aware of the expectations and their rights.

· Explain program expectations and rights to clients and landlords.

· Other duties include calculating rent, determining rent reasonableness and FMR, lead compliance, habitability or HQS inspection, yearly recertification, as well as other regulatory requirements for

the specific housing program.

· Provide specialized services to clients in areas in which the case manager is proficient. This may include domestic violence, youth, veterans, alcohol and other drug addiction, parenting, mental illness, employment, landlord tenant, or SSI/SSDI - SOAR.

· Coordinate appropriate referrals in areas where there are more acute needs or staff do not have specialized skill or training.

· Assist client with move on strategies when no longer in need of the program or time is up.

· Assist in outreach to landlords and identification of new housing units when needed.

· Collect required data, enter required information into HMIS by the end of each week, and other activities and information for performance measurement.

· Complete, maintain, and process pertinent paperwork and records.

· Participate in appropriate community networking, meetings and trainings.

· Develop and maintain an effective body of knowledge regarding resources for clients.

· Complete assigned trainings.

· Participate in assigned meetings.

· Some evening and week-end hours are expected based on client availability.

· Other duties may be assigned.

· Information shared in agency documentation, including but not limited to client interaction, documentation, incident reports, or accident reports demonstrates cooperation, honesty and full factual disclosure.

· Independently assume position functions and integrate knowledge into daily performance.

Qualifications

· Associates degree in Social Work or related field. Bachelor’s degree in Social Work or related field preferred.

· Minimum one or more years’ experience providing case management duties with homeless individuals or in human services.

· Knowledge of mainstream resources and community social services.

· Experience with HMIS or client database is preferred.

· SOAR Certification or bilingual proficiency in Spanish is a plus.

Knowledge, Skills & Abilities

Valid Driver’s License.
Strong communication skills with diverse populations.
Proficiency in database management, spreadsheets, and word processing software.
Demonstrated record of prompt and consistent attendance.
Flexibility to work outside of regular hours when needed.

Work environment/physical demands

· Must be able to lift up to 50#.

· Must be able to bend, stoop, stretch, and climb stairs.

· Frequent local travel for home visits.
