Description of Work
The Physical Activity and Healthy Eating Specialist serves as a subject matter expert supporting the statewide Rural Health Transformation Program by providing training, technical assistance, and guidance on evidence‑based physical activity and healthy food access strategies. The position administers program activities through independent consultation, data interpretation, and analysis of trends related to physical activity and healthy eating outcomes.
Work includes guiding regional Hubs in planning, implementing, and evaluating strategies that increase access to physical activity and healthy foods, while supporting data collection, surveillance, and reporting requirements. This role requires substantial program management knowledge and consistent interaction with internal and external stakeholders. The position operates within a statewide public health environment that emphasizes evidence‑based practice, program coordination, and data‑driven decision‑making.
Key Duties Include:
– Provide training and technical assistance to regional ROOTS Hubs implementing physical activity access, integration goals, and Food is Medicine strategies.
– Guide Hubs in planning and executing evidence‑based physical activity and healthy food access interventions.
– Support Hubs in conducting risk‑factor gap assessments to inform planning and intervention priorities.
– Engage healthcare providers and multidisciplinary teams in strategies that promote increased physical activity and healthy eating.
– Collect, analyze, and interpret data associated with physical activity and healthy eating outcomes.
– Support surveillance and monitoring of physical activity and healthy food access metrics for statewide reporting needs.
– Consult with program partners to interpret trends, research findings, and emerging evidence related to physical activity and healthy food access.
– Provide guidance on programmatic requirements, goals, and elective strategies under the Rural Health Transformation Program.
– Communicate program updates, technical guidance, and analytic findings clearly to internal and external stakeholders.
– Coordinate program activities of moderate complexity while serving as a technical resource across specialized chronic disease domains.

The N.C. Division of Public Health's Chronic Disease and Injury Section, along with local health departments and other community partners, works to reduce death and disabilities through education, policy change and various services. Our goal is to help all North Carolinians develop healthy and safe communities and health systems to prevent and control chronic diseases and to eliminate health inequities.
NC Rural Health Transformation Program is a statewide program led by NCDHHS to transform rural health care delivery, improve health outcomes, and ensure sustainable access to care for nearly 3 million rural North Carolinians. The program is funded over five years by the Centers for Medicare & Medicaid Services (CMS).
The main goals of the program are to:
Catalyze innovative health care models for rural communities by changing the way providers work together to care for patients in rural NC.
Transform the rural care experience, building community-based clinical, behavioral, and social supports close to home.
Create a sustainable rural health delivery system through underlying systems changes in rural workforce pipelines and care team models, and rural provider financial models.
