We're looking for Physical Therapist Assistants to provide home-based therapy throughout Waverly, Watertown, Hanover, Annandale, Kimball, Maple Lake, South Haven, Buffalo, and surrounding communities.

Whether you're interested in building a full-time caseload or committing to as little as two consistent weekdays per week, this is an opportunity to continue doing meaningful hands-on therapy while creating a schedule that leaves room for life outside of healthcare.

Monday Through Friday Means Monday Through Friday

We want this to be clear from the beginning:

No Saturday visits.
No Sunday visits.
No weekend rotation.

Full-Time and Part-Time opportunities are available, with a minimum commitment of 2 weekdays and 12 visits per week.

You're not practicing stairs on equipment in a rehab gym.

As a PTA, you'll work closely with the supervising Physical Therapist to turn the plan of care into meaningful, measurable progress inside each patient's actual living environment.

What You'll Do

Provide physical therapy treatment visits in patient homes.
Implement individualized treatment plans established by the supervising Physical Therapist.
Help patients improve strength, balance, endurance, mobility, and functional movement.
Provide gait and transfer training.
Guide patients through therapeutic exercises and home exercise programs.
Reinforce fall-prevention and safety strategies.
Educate patients, families, and caregivers.
Monitor patient response to treatment and identify changes in functional status.
Communicate patient progress and concerns to the supervising Physical Therapist.
Complete accurate and timely EMR documentation.
Collaborate with nurses, OTs, SLPs, physicians, and other members of the interdisciplinary care team.
Travel between patients within your assigned territory.

Coming From Another Therapy Setting? Your Experience Can Translate

We'd especially like to hear from PTAs with experience in:

Home Health
Outpatient Therapy

Hospital-Based Rehabilitation

Previous home health experience is preferred, but it isn't required.

What We're Looking For

Current Minnesota Physical Therapist Assistant (PTA) license.
Valid driver's license and reliable transportation.
Ability to work independently in a field-based environment.
Strong patient communication and relationship-building skills.
Excellent organization and time-management abilities.
Ability to collaborate effectively with a supervising Physical Therapist.
Home health experience preferred but not required.
Availability for at least 2 weekdays and 12 visits per week.
