Position Overview

Altruist Home Health Care, Inc. is seeking a Physical Therapist (PT) to provide skilled, compassionate, and patient-centered therapy services to individuals receiving care in their homes.

The Physical Therapist will assess patients, develop individualized treatment plans, provide therapeutic interventions, and evaluate progress to help patients improve mobility, function, independence, and overall quality of life.

Essential Responsibilities

Conduct comprehensive physical therapy evaluations and assess patients' functional abilities, mobility, strength, balance, and physical limitations.
Develop, implement, and regularly evaluate individualized physical therapy treatment plans based on physician orders and patient needs.
Establish measurable goals designed to improve functional mobility, independence, safety, and quality of life.
Provide skilled physical therapy interventions in accordance with the patient's plan of care.
Monitor patient progress and modify treatment approaches as clinically appropriate.
Collaborate with RNs, LVNs, physicians, caregivers, and other members of the interdisciplinary healthcare team to promote continuity and coordination of care.
Communicate effectively with patients and families regarding treatment plans, progress, safety, and home exercise programs.
Maintain accurate, timely, and complete clinical documentation, including evaluations, progress notes, reassessments, case conference documentation, and discharge summaries.
Follow physician orders, agency policies, applicable regulatory requirements, OSHA standards, and infection prevention and control practices.
Monitor assigned cases and maintain compliance with Medicare, Medicaid, managed care, and other third-party payer requirements.
Participate in interdisciplinary communication and case conferences as required.
Promote patient safety and identify changes in the patient's condition that require appropriate clinical communication or intervention.
Maintain professional competence through continuing education and ongoing development of clinical skills.
Demonstrate professionalism, dependability, integrity, and a commitment to Altruist's culture of compassionate care.
Represent Altruist Home Health Care, Inc. professionally when interacting with patients, families, referral sources, physicians, and community partners.

Qualifications & Education

Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited/approved program.
Current and unrestricted Physical Therapist license applicable to the state in which services are provided.
Current CPR certification, as required.
Must meet all applicable state, federal, agency, and payer requirements for providing home health services.

Experience Requirements

1–2 years of professional Physical Therapy experience preferred.
Previous Home Health experience is required.
Experience working with patients in a home-based or community setting is strongly preferred.
Strong clinical assessment, documentation, communication, and organizational skills.
Ability to work independently while effectively collaborating with an interdisciplinary healthcare team.
