Full Job Description

Qualifications

Applicant must be a Registered Nurse in the State of Michigan. License must be active and in good standing with the state. Must meet the criteria for a Children’s Mental Health Specialist, Qualified Intellectual Disabilities Professional and Qualified Mental Health Professional. Person must have police and DHS clearance. Must have a minimum of one year post graduate experience working with children and adults with Developmental Disabilities, persons with a Traumatic Brain Injury, and/or Serious Mental Health issue. Additional requirements include: a nonjudgmental approach that reflects cultural competency and sensitivity to the unique needs and diverse experiences of individual families, the ability to engage and maintain clinical relationships with families, the ability to develop collaborative relationships with other providers, effective interpersonal communication skills, the ability to assess and intervene within complex family and community systems, and a willingness to be flexible, patient and creative.

Position Summary

The Nurse Care Manager ensures integrated health care by collaboration, coordination, and a comprehensive approach to healthcare for individuals diagnosed with a severe mental health issue and/or a developmental disability. The Nurse Care Manager serves as a member of the Behavioral Health Home care team.

Specific Responsibilities

Complete Initial and Annual Comprehensive Health Assessments
Participates in the selection of strategies to implement evidence-based wellness and prevention initiatives
Participates in initial care plan development including specific goals for all members
Monitor written goals and objectives and amend when applicable
Communicates with medical providers, subspecialty providers including mental health and substance abuse service providers, long term care and hospitals regarding records including admission/discharge
Provides education in health conditions, treatment recommendation, medications, and strategies to implement care plan goals including both clinical and non-clinical needs
Monitors assessments and screenings to assure findings are integrated in the care plan
Facilitates the use of the PsyGenics electronic health record (EHR) and other Health Information Technology (HIT) to link services, facilitate communication among team members and provide feedback
Monitors and report performance measures and outcomes
Meets regularly with the care team to plan care and discuss cases and exchanges appropriate information with team members in an informal manner as part of the daily routine of the clinic
Excellent communication skills both written and verbal
Demonstrate excellent organizational and planning skills
Fieldwork is required
Attend DWIHN meetings, court appearances, and other required meetings
Participates on the Behavioral Health Home multidisciplinary team
Must be knowledgeable in the Mental Health Code and Medicaid guidelines as well as billing protocols and procedures
Monitor satisfaction with services
Provide resource development to reduce the amount of service gaps
Develop and maintain written records necessary for professional practice as defined in the procedural manual
Provide outreach services to individuals and their families in their respective locales and/or homes at least monthly, or as defined in their IPOS
Provide community education services as assigned
Attend all internal and external meetings, as needed, or directed
Maintain all required licenses and credentials
Assist in the Behavioral Health Program
Maintain productivity standard of great than 50%
Participate in clinical supervision, team meetings and case consultations
Obtain all prior authorizations and applicable coordination of care
Follow PsyGenics, Inc. professional code of conduct
Represent PsyGenics, Inc. in the community
Perform similar or related duties as assigned
