About Kindful Restoration
Kindful Restoration is a community-based nonprofit organization dedicated to helping individuals and families overcome barriers, access essential services and achieve greater stability. We provide case management, reentry support, housing assistance, behavioral health linkages, transportation coordination, community outreach and other supportive services.
We are expanding our Community Health Worker Case Management team throughout California and are seeking dependable, compassionate and organized professionals to serve participants in Riverside, Sacramento, Los Angeles and San Diego.
Position Summary
The Community Health Worker Case Manager provides supportive case management, outreach, advocacy, care coordination and follow-up services to individuals and families who may face barriers to accessing healthcare, behavioral health services, housing, benefits, employment, education and community resources.
This position serves as a trusted connection between participants, Kindful Restoration, healthcare providers, managed care plans, county agencies, housing providers and other community organizations.
Community Health Worker Case Managers may be assigned to work in an office, in the field, remotely or through a combination of work settings. Remote employees must still reside within or near their assigned service area and may be required to attend in-person meetings, participant appointments, outreach activities or community events.
This role may support justice-involved individuals, people experiencing housing instability, individuals with behavioral health or substance use needs and other community members who require assistance navigating complex service systems. The position is based on the duties and qualification standards established in Kindful Restoration’s Community Health Worker job description.
Available Work AssignmentsIn-Office Community Health Worker Case Manager
The in-office Community Health Worker Case Manager provides participant support primarily by telephone, email, video conferencing and electronic case-management systems. Responsibilities include conducting follow-up, coordinating referrals, verifying services, completing documentation and communicating with providers and community partners.
Field Community Health Worker Case Manager
The field-based Community Health Worker Case Manager meets participants in the community, at housing locations, healthcare facilities, partner agencies, correctional settings and other approved service locations. This position requires frequent local travel throughout the assigned region.
Remote Community Health Worker Case Manager
The remote Community Health Worker Case Manager performs case coordination, participant outreach, documentation, referrals and follow-up from an approved remote workspace. Remote work does not eliminate field or in-person responsibilities. Employees must be available to attend required meetings, training, outreach activities and participant appointments within their assigned service area.
Essential Duties and ResponsibilitiesCase Management and Participant Support
- Manage an assigned caseload and maintain regular communication with participants.
- Complete participant assessments and identify needs, strengths, goals and barriers.
- Work with participants to develop individualized service and stability plans.
- Provide outreach, engagement, advocacy, education and ongoing follow-up.
- Build trusting relationships while maintaining professional boundaries.
- Help participants understand available services and participate actively in their care plans.
- Provide encouragement, accountability and practical assistance.
- Monitor participant progress and update service plans when needs or circumstances change.
- Support participants from diverse backgrounds with dignity, patience and respect.
Healthcare and Behavioral Health Coordination
- Connect participants to primary care, mental health services, substance use disorder treatment, dental care and other healthcare providers.
- Assist with appointment scheduling, reminders, transportation coordination and follow-up.
- Communicate with healthcare providers and internal team members to promote continuity of care.
- Help reduce gaps in services for individuals at risk of hospitalization, homelessness, overdose, crisis or recidivism.
- Identify urgent participant needs and report concerns to the appropriate supervisor.
- Follow required procedures for emergencies, mandated reporting and crisis escalation.
Medi-Cal and Benefits Assistance
- Assist participants with Medi-Cal enrollment, activation, renewal and managed care plan navigation.
- Verify Medi-Cal eligibility and managed care plan assignment when required.
- Explain available Medi-Cal programs and services, including Enhanced Care Management and Community Supports.
- Track pending eligibility and service-access issues until they are resolved.
- Assist participants with identification documents, food resources, public benefits and other assistance programs.
- Communicate with managed care plans and service providers regarding referrals, eligibility and authorization issues.
Housing and Community Resource Coordination
- Assist participants with shelter referrals, housing navigation, transitional housing, permanent housing and tenancy-support resources.
- Connect participants with food, clothing, hygiene supplies, employment services, educational opportunities and transportation.
- Help participants develop life skills, wellness goals, employment goals and long-term stability plans.
- Coordinate with Kindful Restoration programs and external providers to prevent duplication or gaps in services.
- Encourage participants to follow through with referrals, appointments and program expectations.
Reentry and Justice-Involved Services
- Support individuals returning to the community after incarceration or other justice-system involvement.
- Assist participants with obtaining identification, benefits, healthcare, housing, employment and supportive services.
- Coordinate with probation, parole, county departments, correctional partners and reentry organizations when authorized.
- Help participants identify barriers that may affect successful community reintegration.
- Maintain respectful and nonjudgmental communication while promoting personal responsibility and accountability.
Documentation and Compliance
- Complete accurate and timely case notes, assessments, referrals, follow-up records and outcome documentation.
- Document participant services in required electronic systems and internal tracking platforms.
- Track participant eligibility, appointments, referrals, housing status, service engagement and outcomes.
- Ensure documentation complies with Medi-Cal, managed care plan, county, grant and organizational requirements.
- Maintain organized and audit-ready participant records.
- Protect confidential information in accordance with HIPAA, privacy requirements and Kindful Restoration policies.
- Meet all documentation deadlines and productivity expectations.
- Use computers, email, calendars, electronic case-management systems and communication platforms accurately and professionally.
Strong computer navigation and documentation skills are vital for this position.
Communication and Team Collaboration
- Participate in case conferences, staff meetings, training and partner meetings.
- Provide supervisors with timely updates regarding participant progress, barriers and urgent concerns.
- Communicate professionally with participants, families, providers, managed care plans, government agencies and community partners.
- Work collaboratively with Kindful Restoration staff across programs and service regions.
- Maintain professional, respectful and solution-focused communication.
- Respond to assigned messages, referrals and follow-up requests within required timelines.
Required Qualifications
- Experience in case management, community outreach, healthcare navigation, behavioral health, housing, peer support, social services, reentry or another community-based program.
- Ability to work respectfully with justice-involved individuals and people from diverse backgrounds and life experiences.
- Strong communication, organization, documentation and follow-up skills.
- Ability to manage multiple participants, responsibilities and deadlines.
- Ability to maintain confidentiality and professional boundaries.
- Strong computer navigation skills and the ability to learn electronic documentation systems.
- Reliable internet access and an appropriate confidential workspace for remote assignments.
- Valid California driver’s license, reliable transportation and current automobile insurance for positions requiring local travel.
- Ability to pass required background checks, screenings and employment-verification requirements.
- Ability to travel throughout the assigned service area when required.
Preferred Qualifications
- Community Health Worker certification.
- Peer Support Specialist certification.
- Substance Use Disorder certification or related behavioral health training.
- Previous case-management or care-coordination experience.
- Experience with Medi-Cal, CalAIM, Enhanced Care Management, Community Supports or managed care plans.
- Knowledge of behavioral health, substance use treatment, housing navigation, reentry services and community resources.
- Experience working with electronic health records or case-management systems.
- Education in social work, psychology, public health, human services, criminal justice or a related field.
- Bilingual skills are strongly valued based on community needs.
- Lived experience with reentry, recovery, housing instability or service-system navigation is valued when appropriately aligned with the position.
Community Health Worker Certification
Applicants do not need to already hold a Community Health Worker certificate to apply.
Kindful Restoration provides Community Health Worker certification training to qualified employees who are not currently certified.
Applicants who are not CHW certified must have relevant experience or comparable qualifications, which may include:

- Higher education in a related field

Employees may work with participants experiencing complex medical, behavioral health, housing, legal, financial and social-service needs.
Remote employees must maintain a private workspace that protects participant confidentiality and must comply with all attendance, availability, productivity, documentation and communication requirements.
Physical Requirements

Schedule
Full-time schedules are available. Work hours may vary depending on program assignment, service region and participant needs. Some evening or weekend availability may be required for field assignments, outreach events or special program activities.
