Role Overview:

The Clinical Operations Coordinator is a Registered Nurse responsible for supporting the clinical and operational infrastructure that ensures high-quality, compliant, and financially accurate patient care across the organization. This role provides oversight of clinical documentation and chart integrity, regulatory compliance, quality assurance, EMR administration, authorization accuracy, and billing integrity. The Clinical Operations Coordinator works closely with clinical leadership and cross-functional teams to identify documentation and compliance gaps, strengthen workflows, and ensure patient records remain accurate, complete, and audit-ready. The position also supports clinical education and corrective action initiatives designed to improve documentation quality and prevent recurring deficiencies. Through proactive monitoring, collaboration, and process improvement, this role serves as an important connection between clinical care, intake and authorization, quality assurance, operations, and billing.

Key Responsibilities and Duties:

1. Quality Assurance & Chart Integrity Oversight

This position will be overseeing a team that is responsible for agency-wide document management, quality assurance oversight, and chart integrity, ensuring all clinical, operational, and regulatory documentation is accurate, complete, compliant, and audit-ready in accordance with home health industry standards and regulatory requirements and in compliance with DOL requirements.

2. Regulatory Compliance & Survey Readiness

This role is responsible for ensuring the agency maintains full compliance with all applicable federal, state, payer, and regulatory requirements, including but not limited to, Plan of Care management, supervisory visits, management of physician orders, notification requirements for physician. The position also monitors regulatory changes and coordinates with manager to communicate updates to leadership to ensure policies, procedures, and documentation practices remain current. Additionally, the role collaborates with department manager and clinical leadership to identify compliance risks and improve documentation quality, maintains trackers and evidence of compliance activities; leads quality assurance reviews and trend analyses related to clinical documentation and regulatory performance; and promotes a culture of compliance, accountability, and continuous improvement throughout the organization.

3. Clinical Training and Corrective Action Plans

This role is responsible for ensuring regulatory compliance and survey readiness by conducting routine chart audits and reviewing other team members' audit responses. Responsibilities include overseeing chart integrity, developing and implementing corrective action plans to address documentation deficiencies and audit findings, and providing clinical education and training to improve documentation accuracy, regulatory compliance, and quality of care.

4. Administrative management of EMR system

Responsible for the administrative management of the electronic medical record (EMR) system, including oversight of patient census, current physician information, discharges, initial assessments, and start-of-care processes. Ensures the accurate and timely entry, updating, and maintenance of patient records, while preserving data integrity and supporting compliance with clinical, regulatory, and organizational requirements.

5. Oversight and management of billing integrity

Provides oversight and management of billing integrity to ensure accurate claim submission, regulatory compliance, reimbursement optimization, accurate authorization input, and timely resolution of billing discrepancies. Collaborates with clinical and billing teams to address documentation and reimbursement issues and maintain compliance with payer and regulatory requirements.

Qualifications:

Current, unrestricted Registered Nurse (RN) license required.

Minimum of 2 years of nursing experience; home health, home care, or community-based healthcare experience strongly preferred.

Experience with home health clinical documentation, Quality Assurance (QA), chart auditing, or regulatory compliance strongly preferred.

Intake and Authorization experience preferred, including authorization management, referral/intake workflows, and ensuring authorization accuracy within the EMR.

Working knowledge of Plans of Care, physician orders, start-of-care documentation, supervisory requirements, discharges, and other home health clinical documentation standards preferred.

Experience working within an Electronic Medical Record (EMR) system, including maintaining patient records, documentation workflows, and data integrity.

Understanding of how clinical documentation impacts billing integrity, reimbursement, authorization compliance, and claim submission.

Demonstrated ability to identify documentation deficiencies, conduct or support chart audits, and implement corrective actions.

Strong attention to detail, critical thinking, organization, and ability to manage multiple competing priorities and regulatory deadlines.

Strong written and verbal communication skills with the ability to collaborate across clinical, intake/authorization, billing, and operational teams.
