Full Job Description

Health Coach — Part-Time, Virtual | Cultivate Health Functional Medicine

About Us Cultivate Health is a functional medicine practice helping people resolve the root causes of their health concerns through evidence-based, integrative care. We combine functional and conventional medicine with education, community, and compassionate accountability, and we practice what we teach. As a team member, you'll help aid in the vision of the company moving forward, truly creating long-lasting change in health and wellness, not just for the patients we see, but for the communities and food systems we're all connected to.

About the Role This role is centered on ongoing one-on-one and group coaching for patients moving through their health journey, plus connecting with prospective patients through short discovery calls. Most work is virtual, serving telehealth patients across Wisconsin, Maryland, and Virginia, with occasional in-person opportunities at our Wisconsin farm celebration events..

What You'll Do

Lead 1:1 coaching visits (1–2x/month per patient) and group coaching sessions (1–2x/week)
Support patients through the phases of their health journey
Maintain a "roll call" of active patients so our team always knows how to support them
Handle discovery calls (~10/week, 15 minutes each) — connecting with prospective patients, understanding their concerns, and sharing our approach
Help plan patient and community events (2–4 hrs/month)
Document visits in our EHR (Cerbo)

Position Details

Virtual, ~20 hours/week, flexible schedule
$25–$30/hour, with a raise after the probationary period and performance-based bonuses
Room to grow into full-time hours and a leadership role as our team expands

Health coaching certification (FMCA, IIN, NBC-HWC) or a relevant degree preferred; candidates working toward certification are welcome to apply
