Full Job Description

Overview
We are seeking a dedicated and compassionate Care Navigator to join our team. In this vital role, you will serve as a guiding resource for patients, helping them navigate complex healthcare systems. You would be responsible for coordinating and continually monitoring the care of patients with chronic conditions in our chronic care management (CCM) program, collaborating closely with physicians, nurses, pharmacists, and community partners to help ensure they meet their health goals.

Responsibilities:

Conduct comprehensive patient assessments to develop individualized care plans
Collaborate with interdisciplinary care teams, including hospitals and primary care providers
Monitor patient health status and facilitate follow-up care
Educate patients and families about self-management, medications, and available resources
Maintain accurate and timely documentation in compliance with regulatory standards
Advocate for patient needs and coordinate services to ensure continuity of care

You are

Passionate about patient-facing Chronic Care navigation
Proud of your organized patient care coordination skills
Medical Assistant or health education background strongly preferred
Confident self-starter, able to be highly productive and self-structured
Comfortable in fast-paced, change-oriented environment
Amiable, active team player
Understand concepts of, and experience with, active listening skills
Expert keyboard skills; Fluency with EHR interface
Experience with diverse patient populations; culturally competent communication skills
Excited about patient-centric, resource- finding, and solution-based approaches to care
Must maintain a HIPAA-compliant home workspace, including a dedicated, private area to ensure confidentiality.
Reliable home Wi-Fi connection
Strong organizational skills and attention to detail are required to manage multiple tasks efficiently

What Success in This Role Looks Like for You

The Nurra Health Care Partner initiates patient relationships with compassion, advanced communication skills, and the curiosity to improve their health outcomes. You are prepared for and responsive to patients experiencing distress, frustration, and the range of emotions arising from serious chronic health issues. You are motivated to discover their goals and needs, address gaps in health knowledge, support patient agency and autonomy, and ensure seamless speciality care coordination.

You possess advanced communication skills when talking with patients about personal matters, navigating difficult conversations, and anticipating patient needs. Prior experience working with complex patient populations such as elder services or high needs populations is a plus but not required. Critical to your success is bringing highly effective communication with strong interpersonal skills and the ability to work collaboratively and proactively within a team of colleagues, partner PCPs, and office staff. You are a self-starter, well organized, and are able to work autonomously, efficiently and with accountability.

Core Responsibilities

Efficiently and effectively manage a panel of approximately 150 patientsof varying acuity for whom you are a high-touch chronic care coordinator, point person and health ally.
Establish engagement and trust effectively with distressed or reticent patients.
Develop collaborative relationships with patients and their families/caregivers.
Implement diagnosis-specific Plans of Care targeting improvement of high- risk medical conditions, closing care gaps, and advancing patient comfort and trust.
Strong accountability to patients’ goal achievements and metric outcomes.
Engage in patient-empowered self-management, health information, medication reconciliation and adherence, health confidence tracking, sourcing relevant resources with AI assistance
Maintain accurate and timely documentation
Active team participation with willingness to roll up your sleeves, assuming team-based growth projects, and the ability to maintain an iterative approach to workflows and expectations.
Real-time availability during standard business hours; consistent, real-time availability at your workspace throughout the day

Pay: $22.00 - $24.00 per hour
