Job Summary
OwnaHealth is a virtual clinic on a mission to reverse Type 2 Diabetes and obesity through food, not pharmaceuticals. We serve underserved populations — people with limited access, limited means, and often very complicated cases of uncontrolled diabetes. Using a nutrition-first approach grounded in high-fat, low-carbohydrate and ketogenic principles, we help patients eliminate insulin use, reduce A1c levels by over 2 points, and regain control of their lives — even in the most challenging circumstances.

We are seeking a dynamic and compassionate Telehealth Nurse Practitioner (Endocrinology) to join our innovative healthcare team. In this role, you will deliver expert endocrine care remotely, providing comprehensive assessments, diagnosis, and management for patients with hormonal and metabolic disorders. Your expertise will help improve patient outcomes through virtual consultations, education, and collaborative care planning. This position offers an exciting opportunity to leverage your clinical skills in a flexible, technology-driven environment dedicated to accessible, high-quality healthcare.

Responsibilities

Provide virtual endocrinology care to patients via video and phone through our AI-powered Mobile App.
Manage a small caseload of medically complex patients with uncontrolled Type 2 Diabetes, obesity, and related cardiometabolic conditions.
Lead medication management and deprescribing — including titration and discontinuation of insulin, oral antihyperglycemics, anti-hypertensives, and other comorbidity medications as patients improve.
Order, interpret, and act on labs, CGM data, and remote monitoring data (BG meter, scale, BP cuff, ketone meter) to guide clinical decisions.
Develop and adjust individualized treatment plans rooted in low-carbohydrate and ketogenic nutrition protocols.
Collaborate closely with care coordinators, coaches, dietitians, and supervising endocrinology to ensure comprehensive care.
Quickly establish rapport and therapeutic alliances with a diverse, often underserved patient population.
Utilize motivational interviewing and other evidence-based techniques to support sustained behavior change.
Consistently meet clinical and program metrics while maintaining high-quality, compassionate care.
Complete all clinical documentation in a timely manner.

Qualifications

Active Nurse Practitioner (NP) or Physician Assistant (PA) licensure in the State of New York, in good standing.
Clinical experience managing Type 2 Diabetes, obesity, and cardiometabolic comorbidities.
Strong interest in low-carbohydrate / ketogenic nutrition, lifestyle medicine, and medication deprescribing.
Experience and/or genuine interest working with underserved or high-risk populations.
A compassionate, non-judgmental, and empowering approach to patient care.
Comfort with telemedicine platforms and electronic health records.
A HIPAA-friendly workspace with high-speed internet for secure patient conversations.
Demonstrated ability to adapt communication techniques to meet the needs of a diverse patient population.
Flexible schedule with a commitment to a minimum of 20 hours per week.
Must live in the United States.

Pay: $70.00 - $80.00 per hour
