Nurse Administrator / Office Manager

Home Health Care Agency– Rhode Island

Position Summary

The Nurse Administrator serves as the clinical and regulatory lead for a home health care agency in Rhode Island and is responsible for ensuring compliance with all applicable Rhode Island Department of Health (RIDOH) regulations. This position oversees client assessments, care planning, caregiver compliance, and clinical documentation while also supporting office operations and maintaining a professional environment for clients, caregivers, and visitors.

The Nurse Administrator acts as the licensed nurse listed on the agency's RIDOH license and works closely with management to ensure the delivery of high-quality, compliant home care services.

Essential Duties and Responsibilities

Regulatory Compliance & Clinical Oversight

Serve as the Nurse Administrator designated on the agency's Rhode Island Department of Health license. Assist in maintaining compliance with all RIDOH regulations and agency policies. Monitor caregiver files and documentation to ensure all licensing, certifications, training, health records, and required compliance documents remain current. Participate in agency audits, surveys, and compliance reviews as needed. Assist management in implementing corrective actions and maintaining regulatory standards.

Caregiver Recruitment, Training & Development

Conduct Certified Nursing Assistant (CNA) interviews as needed. Evaluate candidate qualifications and make hiring recommendations. Conduct and facilitate new caregiver orientations. Provide guidance and education regarding agency policies, procedures, documentation requirements, and standards of care. Serve as a clinical resource for caregivers and administrative staff.

Client Assessments & Care Planning

Conduct all initial client assessments and service consultations. Develop individualized care plans based on client needs and preferences. Complete all required documentation and client service agreements. Enter contracts, assessments, and care plans into WellSky Personal Care. Conduct routine client supervisory visits and reassessments in accordance with agency policy and Rhode Island regulations. Update care plans as client conditions or service needs change. Communicate assessment findings and recommendations to appropriate team members.

WellSky Management

Maintain accurate and timely documentation within WellSky Personal Care. Manage and monitor prospective client records within WellSky. Track assessment status, pending admissions, and client onboarding activities. Ensure all client records are complete, accurate, and compliant with agency standards.

On-Call Responsibilities

Participate in the agency's after-hours on-call rotation. Respond to emergency situations requiring clinical or managerial intervention. Provide guidance and support to caregivers and clients during after-hours emergencies. Coordinate with management and scheduling staff as needed to resolve urgent issues.

Office Management & Administrative Support

Serve as the primary in-person office representative during business hours. Maintain a professional, welcoming, and organized office environment. Ensure brochures, marketing materials, and office supplies are stocked and readily available. Monitor office appearance, including common areas, reception spaces, and conference rooms. Coordinate routine office needs, including plant care, visitor accommodations, and general upkeep. Ensure office security procedures are followed, including accountability for office keys and shared resources. Communicate facility maintenance needs or concerns to management promptly. Coordinate with the Remote Scheduler to select and recognize a monthly Employee of the Month. Prepare and mail Employee of the Month recognition gift cards and related acknowledgments in a timely manner. Monitor inventory of caregiver and office supplies, including personal protective equipment (PPE) such as gloves and masks. Notify the Operations Manager when supplies require replenishment and assist with maintaining adequate inventory levels. Support special projects and operational initiatives as assigned.

Qualifications

Current unrestricted Registered Nurse (RN) license in the State of Rhode Island. Minimum of one (1) year of nursing experience preferred. Previous experience in home care, healthcare administration, case management, or community-based care preferred. Strong understanding of Rhode Island home care regulations and compliance requirements. Excellent organizational, communication, and interpersonal skills. Proficiency with electronic health record systems; experience with WellSky Personal Care preferred. Ability to work independently and manage multiple priorities. Valid driver's license and reliable transportation for client visits and assessments.

Physical Requirements

Ability to travel locally for client assessments and supervisory visits. Ability to sit, stand, walk, bend, and lift up to 25 pounds as needed. Ability to use standard office equipment and computer systems.

Reporting Structure

Reports To: Operations Manager / Franchise Owner

Supervises: Clinical and caregiver compliance activities as assigned.
