Full Job Description

Job Summary:

The primary function of the Hospice Liaison is to serve as the primary relationship builder between the Agency and referral sources, healthcare providers, community organizations, patients, and families. This role is responsible for developing and promoting hospice services, increasing community awareness, and fostering strong referral partnerships while maintaining the highest standards of ethical practice, quality patient care, and regulatory compliance. The Hospice Liaison works collaboratively with the interdisciplinary team to support agency growth while maximizing human, financial, and organizational resources.

Essential Functions:

Serves as the Agency representative within the community by developing and maintaining strong relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, case managers, social workers, and other referral sources.
Identifies community needs and provides recommendations to leadership regarding program development, service expansion, staffing needs, and strategic growth opportunities.
Collaboratively develops and maintains policies and initiatives specific to new programs and community outreach efforts.
Demonstrates an in-depth knowledge of, and ensures compliance with, all local, state, and federal laws and regulations relating to hospice marketing and patient referrals.
Assists in directing agency growth initiatives through collaboration with administrative and supervisory personnel.
Partners with the Interdisciplinary Team to support safe, compassionate, and effective patient and family care.
Plans and participates in community events, educational presentations, networking opportunities, and professional organizations to promote the Agency's mission and services.
Establishes and maintains a public relations program that effectively communicates the Agency's services and fosters positive working relationships with physicians, healthcare professionals, and community partners.
Maintains accurate documentation of referral activities, community contacts, and marketing efforts in accordance with Agency policies.
Meets established marketing goals and contributes to the Agency's overall census growth through ethical relationship-building and exceptional customer service.
Carries out other duties as assigned by the Board of Directors.

Job Qualifications:

Education: Graduate of an accredited college or university is preferred.

Experience: Previous experience in healthcare marketing, hospice, home health, physician relations, business development, or a related healthcare field is preferred.

Skills:

Ability to establish, develop, and maintain effective professional relationships with referral sources, physicians, community partners, patients, and families.
Excellent verbal, written, interpersonal, presentation, and customer service skills.
Strong organizational and time management skills with the ability to work independently and prioritize multiple responsibilities.
Ability to work under a variety of conditions in healthcare facilities, physician offices, hospitals, and community settings.
Ability to travel locally and maintain a flexible work schedule, including occasional evenings or weekends for community events.
Requires the ability to handle stressful situations with professionalism, discretion, and courtesy while meeting deadlines and Agency needs.
Must possess reliable transportation, maintain a valid driver's license and auto insurance, and be willing to travel throughout the Agency's service area.
