Full Job Description

Who We Are

At Complex Care Center, we simplify the complexities of care by putting patients first. We support individuals with chronic and complex conditions through coordinated, compassionate, and accessible primary care. As a Patient Support Specialist, you will be part of a remote, patient-facing team that serves as a key point of contact for our patients, facilities, and clinical staff. Our approach is grounded in evidence-based care models that improve health outcomes and quality of life.

Position Overview

This is a remote call center–based role focused on patient communication, scheduling, and care coordination. Patient Support Specialists manage a high volume of inbound and outbound calls, support appointment scheduling, and ensure accurate documentation to help our clinical teams deliver timely care.

Responsibilities

Handle inbound and outbound patient calls in a professional, courteous, and efficient manner
Schedule, reschedule, and confirm appointments across multiple providers and locations
Provide general information to patients, caregivers, and facility staff while maintaining HIPAA compliance
Coordinate and process patient referrals
Perform accurate and timely data entry in the electronic health record (EHR)
Monitor and manage patient messages, faxes, and other incoming communications
Collaborate closely with clinical and administrative teams to support continuity of care
Deliver a consistently positive patient experience through clear communication and problem-solving

Qualifications

Strong verbal and written communication skills
Comfortable working in a phone-based, high-volume call environment
Ability to multitask, prioritize, and navigate multiple systems efficiently
Bilingual in English and Spanish is a plus
Reliable internet connection and ability to work independently in a remote setting
Healthcare, call center, or patient-facing experience preferred
