ALAMEDA FAMILY SERVICES

Alameda Family Services is a progressive, non-profit agency that provides clinically sophisticated, comprehensive mental health and early childhood educational services to individuals and families in the City and County of Alameda through several programs: Early Childhood Family Support, School Based Services, Crisis Services, and Clinic and Community Based Behavioral Health Care Services. OVERALL RESPONSIBILITIES:

The Intensive Care Coordinator (ICC) is responsible for providing support to youth and their families through creating an individualized care plan that addresses goals to support their wellbeing, such as access to resources, and managing systems such as truancy, foster system, juvenile probation, and/or other needs. The ICC is responsible for engaging and assisting youth, caregivers, and other support persons in forming a Child-Family Team (CFT). The Child-Family Team works with identified support persons from natural (friends, extended family, neighbors, community members) and formal system partners (child welfare, education, juvenile probation, etc.) to comprehensively address the child and family’s identified goals, and ensure that the activities/interventions are coordinated to support successful and enduring change consistent with the Child-and-Family Plan. The coordinator will be an integrated member of the community and is also responsible for collaborating with mental health therapists, the school district, and other formal systems.

JOB DUTIES:

Provide a full range of culturally sensitive, strength-based case management, advocacy, and linkage services to clients and families.
Maintain a full caseload of 16-18 clients.
Facilitate Child and Family Team meetings with the child, family, informal supports, mental health, and other system providers involved, working as a team to meet child and family goals.
Provide a full range of ICC services to identify client needs and gaps in services as well as to facilitate care planning and the coordination of services, including urgent services, for foster youth and all youth requiring intensive care coordination.
Complete accurate and timely documentation of services and maintain all records in compliance with Medi-Cal and all legal, ethical, and professional laws.
Complete all reports and data tracking associated with the position and services.
Coordinate service delivery with other service providers and county partners.
Participate in partner-facilitated meetings such as COST, SARB/SART boards, and other meetings as assigned.
Attend and participate in a variety of staff and program-related meetings, as well as professional development trainings.
Attend and participate in weekly supervision meetings.
Other duties as assigned by the Division Director.

QUALIFICATIONS, SKILLS AND EXPERIENCE:

BA with relevant experience; or MA in psychology, counseling, social work, or a social service-related field.
Minimum two (2) years of successfully providing youth development and/or case management services to children and youth.
Basic understanding of positive youth development, motivational interviewing, and trauma-informed care models of service.
Experience working with ethnically and socio-economically diverse youth and families.
Embraces a strength-based approach to clients, their families, and systems.
Knowledge of unique stressors and needs impacting children, youth, and adolescents.
Ability to work independently and provide services off-site within the community.
Ability to effectively organize and prioritize multiple tasks.
Ability to work independently and as part of a team.
Strong organization skills and ability to implement systems. Skills in record keeping and organization.
Multi-language capacity desired.
