Family Health Services of Darke County is pleased to announce an opportunity for a School-Based Nurse (RN/LPN). This position provides professional nursing services within our School-Based Health Centers, working collaboratively with providers, school personnel, students, families, and other healthcare team members to promote health, wellness, and academic success.
This is an excellent opportunity for an RN/LPN who enjoys working with children, adolescents, and adults in a collaborative healthcare environment while following the school calendar.
The School-Based RN/LPN provides direct patient care to students, school employees, and other eligible patients receiving services through Family Health’s School-Based Health Centers. Responsibilities include assessing patients, coordinating care with providers and school personnel, administering medications and immunizations, assisting with examinations and procedures, providing patient education, and maintaining accurate documentation in the Electronic Medical Record (EMR). The RN/LPN serves as an advocate for patient health while supporting Family Health’s mission of Building Healthy Lives Together.
- Seasonal Position: Approximately Mid-August through Mid-June
- Schedule generally follows the participating school district calendar.
- Hours are primarily Monday through Friday during the school day.
- Attendance may be required for teacher workdays, staff training, summer planning, or other Family Health operational needs
- Current Ohio RN/LPN Nurse license in good standing.
- Current CPR certification.
- Strong assessment, communication, and organizational skills.
- Ability to work independently while collaborating with providers, school personnel, and interdisciplinary team members.
- Pediatric, family practice, community health, or school nursing experience is preferred but not required.
- Provide direct nursing care to students, school employees, and other eligible patients seen in the School-Based Health Center.
- Assess, plan, implement, and evaluate nursing care for pediatric, adolescent, and adult patients.
- Perform comprehensive nursing assessments and identify patient needs requiring provider intervention.
- Perform nursing assessments and accurately document patient encounters in the EMR.
- Administer medications, immunizations, injections, and treatments within RN scope of practice.
- Assist providers with examinations, procedures, and treatments.
- Coordinate referrals, prior authorizations, and follow-up care.
- Recognize changes in patient condition and respond appropriately to urgent or emergent situations.
- Educate patients and families on medications, disease prevention, chronic disease management, nutrition, and healthy lifestyles.
- Collaborate with school personnel and Family Health providers to support the health and wellness of both students and school staff.
- Maintain compliance with HIPAA, OSHA, Family Health policies, and nursing standards.
