The AHH Community Liaison will play a vital role in ensuring seamless coordination and quality care for referral sources and patients across different healthcare settings. This will require a blend of healthcare background, communication skills and organizational abilities.
Role
· Community Outreach:
o Establish and maintain relationships with local organizations, healthcare providers, senior centers, and community leaders to promote the GUIDE program.
o Organize and participate in community events, workshops, and educational sessions to raise awareness about dementia care and personal care services.
· Program Advocacy:-
o Serve as the primary ambassador for the GUIDE program, effectively communicating its mission, benefits, and available resources to the community.
o Act as a resource for families and caregivers seeking guidance on dementia care services and in-home primary care needs.
o Complete needed assessments prior to alignment.
· Referral Management:
o Work with referral sources to identify individuals in need of in-home primary care.
o Work with referral sources to identify individuals in need of personal care services for both AHH and Adult Healthcare Solutions, ensuring smooth transitions into the GUIDE program.
o Maintain accurate records of referrals and follow up to ensure satisfaction and service quality.
· Account Management:
o Collaborate with internal teams to ensure alignment of outreach efforts with agency goals.
o Provide regular updates to management on outreach activities, community needs, and program outcomes.
Responsibilities
· Responsible for building a rapport with healthcare professionals (community based).
· Responsible for interacting and assisting with referrals from healthcare professionals in medical clinics, physicians/APPs who have referral potential, group/care homes, and other senior/healthcare community referrals.
· Responsible to educate and work with above entities to assure a smooth admission process is followed and the referral source is informed during the process.
· Intake of new patient information, including insurance verification, and scheduling as needed.

Qualifications and Education Requirements
· Education: A bachelor's degree in nursing, healthcare administration, or a related field is required. Individuals with industry experience will be considered.
· Communication Skills: Strong interpersonal, verbal, and written communication skills are essential for effectively mediating between different stakeholders.
· Problem-Solving Abilities: Ability to assess situations, identify needs, and develop appropriate solutions to meet patient and facility requirements.
· Organizational Skills: Excellent organizational and time-management skills to manage multiple tasks, prioritize responsibilities, and meet deadlines.
· Knowledge of Healthcare Systems: Understanding of healthcare systems, regulations, insurance, and reimbursement processes is valuable.
Preferred Skills
· Networking and Relationship-Building: Ability to establish and maintain relationships with healthcare professionals, patients, families, and external stakeholders.
· Computer Proficiency: Proficiency in using healthcare software and electronic medical records systems.
Additional Notes
· The Community Liaison/Account Manager will play a vital role in ensuring seamless coordination and quality care for patients across different healthcare settings, requiring a blend of clinical expertise, communication skills, and organizational abilities.
· The position is required to be on site in office and satellite sites as needed Monday through Friday 8:30am-5pm with the opportunity for remote work once established and productivity minimums are met. Weekends as needed.
· Employee must meet the hold current driver’s license and be in good standing.
