Full Job Description

Job description

We are seeking experienced QMHPs who are proactive, community-oriented, and capable of independently building and maintaining a caseload. Preference will be given to candidates who already have established referral relationships within the community and experience conducting outreach/networking with schools, agencies, hospitals, probation offices, and other referral sources.

This role is ideal for self-motivated professionals who are comfortable engaging in community outreach, professional networking, and client engagement efforts to support program growth and continuity of care.

This position is best suited for clinicians who are entrepreneurial, resourceful, and comfortable participating in outreach and relationship-building efforts within the community.

This position involves the provision of an array of counseling and educational services.

Duties and Responsibilities

Provide one-on-one behavioral support services to clients
Use good interpersonal communication skills to gain client cooperation and resolve conflicts.
Interact with all clients in accordance with the individual service plan and the therapeutic process.
Assess, evaluate and monitor the safety of the client and others at all times.
Exercise appropriate intervention in accordance with the least restrictive alternative to protect client from harming self or others.
Coordinate closely with other staff and community partners to ensure the appropriateness of treatment modalities and services being provided.
Serve as clinical liaison to referral sources and parents regarding progress and needs; to include attendance of FAPT meetings and court hearings as necessary.
Develop and implement plans for discharge including mobilization of resources for ongoing maintenance, rehabilitation & recovery.
Maintain high quality documentation of all case work, completed in a timely manner and consistent with outlined program policies and licensure requirements.
Notify Supervisor of any significant client medical complaint(s), unusual events, accidents, medical problems, property damage or maintenance concern, etc.
Provide support and linkage to community resources for participants
Monitor mental health symptoms, compliance with medication regimen, health nutrition and physical condition of participants.
Establish and maintain positive relationships with other agencies involved in the care of participant
Promote the program in a positive manner to elicit referrals and maintain census and growth of the program
Ensure that every Incident Report is completed immediately after the incident and submitted to the direct supervisor or Director the same day
Other duties as assigned

Job Requirements

Be available to provide services during the day, evening, weekends and holidays. Provide and maintain an acceptable driving record.
Have access to and ability to use a computer, printer, internet services, and other necessary items and equipment to produce electronic program documentation/paperwork.
Maintain communication with Program Director and Clinical Director through e-mail and cell phone usage on a daily basis.

Knowledge, Skills and Ability

Ability to maintain a flexible schedule.
Ability to manage multiple priorities simultaneously.
