Full Job Description

The Chattanooga Housing Authority (CHA) seeks a qualified individual for the full-time position of Case Manager. The deadline to submit applications is Monday, July 27, 2026.

Position Description

The Case Manager conducts activities to provide service coordination and/or case management services to the frail and at-risk elderly and disabled residents of the CHA. This individual will develop, review, and revise appropriate plans of care that are focused on assisting residents to remain in their own homes as long as possible by linking them with home and community-based services. To the extent possible, assures goals and objectives are met or exceeded. When goals and objectives cannot be met, develops strategies that will progress toward meeting the goals and objectives.

Essential Duties and Responsibilities

The statements contained herein reflect general details to describe the principal functions of this job, the level of knowledge and skill typically required and the scope of responsibility, but should not be considered an all-inclusive listing of work requirements. Individuals may perform other duties as assigned including work in other functional areas to cover absences or relieve others, to equalize peak work periods or otherwise balance the work load.

Directs and conducts social service needs screenings and assessments for CHA elder residents. Determines services that promote residents’ well-being and independent living that meet goals and needs of residents and their families.

Formulates, recommends, and implements services plan to affordably and effectively address each client’s needs and coordinates with CHA staff and other services providers. Draws on knowledge of area service landscape and utilizes any available CHA resources.

Monitors and documents the progress and success of the plans, communicating the status to the Community Manager and the Administrator of Resident Engagement, and revises the plans as necessary.

Used established evaluation metrics and processes for the elder social services program to ensure maximum benefit/cost for CHA and its residents. Maintains complete, orderly, and program-compliant documentation of activities and expenses.

Conducts outreach (written materials, meetings and events, internet-based resources) to elder residents to inform them of the possibilities for assistance, and to hear and be able to address residents’ concerns and needs.

Understands and operates within applicable HUD, federal, state, and local rules and regulations impacting assigned programs.

Networks with various agencies, educational institutions, and community organizations to obtain resources to expand and improve services.

Provides technical assistance for accessing and using resources and information.

Conducts home visits as needed.

10. Researches and prepares grants to secure additional funding streams for the Program.

Assists in preparing annual budget for Program staff and activities. Prepares budget reports; categorizes and records costs; operates program within agreed budgetary parameters.

Exhibits and encourages behaviors that uphold CHA’s core values.

Keeps abreast of HUD, federal, state, and local rules and regulations impacting assigned programs and makes recommendations to the Executive Director.

Establishes and maintains effective working relationships with the CHA Board, employees, residents, participants, landlords (as applicable), contractors, as well as city, state, and federal officials and works harmoniously with community stakeholders.

Attends and/or participates in various meetings related to the CHA as required.

Reports consistently to assigned work location(s) in accordance with established work schedule. Must be reliable.

Provides outstanding customer service by responding to phone calls, emails, and written correspondence in a timely, efficient, and appropriate manner.

Handles confidential matters with discretion.

Performs other duties as directed.

Required Knowledge, Skills and Abilities

Knowledge of principles and practices involved in social service programming designed to develop well-being and self-sufficiency among elder residents of low income.

Ability to understand and carry out policies, regulations, and procedures as set forth by CHA.

Knowledge of general office software: operating system usage including concepts of network file storage; word processing and spreadsheet software at the basic level; internet browser and internet resource usage; maintenance of computer and network resource security.

Ability to communicate and build rapport with people from a broad range of ethnic and socio-economic backgrounds.

Ability to operate commonly used office equipment, especially the use of personal computers for word processing and spreadsheet applications, scanners, copiers, and fax machines.

Strong interpersonal skills and effective communication ability, both orally and in writing.

Ability to work independently and in-group settings and use sound judgment in decision-making and problem solving.

Ability to mediate disputes among CHA employees, residents, and other parties; responds quickly to issues or questions.

Ability to work productively in a variety of working conditions and environments.

Bilingual in English/ Spanish is desirable.

Minimum Education, Training, and/or Experience

A high school diploma/GED is required; graduation from an accredited college or university with an emphasis in social work is preferred.

One years of experience in administrative support work, office management or related field is preferred.

Any equivalent combination of education, training, and experience, which, in the sole determination of the CHA, provides the required knowledge and abilities, may be considered sufficient.

Physical/ Mental Requirements

Level of manual dexterity sufficient to allow for operation of office equipment.

Ability to move, handle, or lift small objects around desk area.

Ability to inspect all dwelling units, buildings, systems and grounds on the site.

Special Requirements

Must possess a valid driver’s license, have reliable transportation with required liability insurance, and be insurable under CHA’s automobile insurance.

Criminal background checks and drug screening will be performed on all applicants.

Must be bondable.
