Hospice RN Case Manager

$80,000–$95,000/Year · Monday–Friday · Albuquerque, NM

Are you a registered nurse ready to make a real difference in the lives of patients and families facing end-of-life care? Lilycare Hospice is hiring a full-time Hospice RN Case Manager to join our growing Albuquerque team providing compassionate hospice care throughout the community.

Schedule: Monday–Friday, 8:00 AM – 5:00 PM. On-call and holiday rotation included. Location: Albuquerque, NM

What You'll Do

As a Hospice RN Case Manager, you'll carry your own caseload across the Albuquerque metro area, delivering hands-on hospice nursing care wherever patients call home — private residences, assisted living, and skilled nursing facilities. You'll work alongside the Clinical Director and a full interdisciplinary team: social workers, chaplains, aides, and a hands-on Medical Director.

Manage your own caseload of hospice patients with real continuity — you build the relationships, you shape the plan, you see it through
Build and maintain each patient's Plan of Care alongside the patient, family, attending physician, and IDT
Develop pain and symptom management plans using pharmacological and non-pharmacological approaches; order medications and DME using the hospice formulary
Coordinate hospice care across the IDT — HHAs, social workers, chaplains, volunteers, and ancillary services
Lead patient and family education on disease progression, comfort care, and what to expect
Participate in weekly IDT meetings and contribute to hospice clinical quality improvement
Cover on-call shifts per the rotation

What You Bring

Required

Active RN license valid in New Mexico (NM single-state or NLC multistate compact)
Associate's Degree in Nursing (ADN), Nursing Diploma, or BSN from an accredited program
Minimum 1 year of RN experience in med-surg, acute care, home health, or hospice
Current CPR/BLS certification (or willingness to obtain within 30 days)
Valid driver's license, current auto insurance, and reliable transportation

Preferred

BSN
Prior hospice or home health case management experience
CHPN (Certified Hospice and Palliative Nurse) certification
HomeCare HomeBase (HCHB) EMR experience
NLC multistate compact license

Schedule

Full-time, Monday–Friday, 8:00 AM – 5:00 PM
On-call and holiday rotation included
Territory: Albuquerque metro
Based out of our Albuquerque, NM office
Reports to the Clinical Director
