Description:

GENERAL STATEMENT OF FUNCTIONS

Responsible for assisting the Program Director in maintaining quality services, preparing reports, attending intake and exit meetings, coordinating the intake of Habilitation clients, and assisting with employment preparation functions.

Requirements:

SPECIFIC DUTIES AND RESPONSIBILITIES

Assists with ensuring compliance with all regulatory, licensing, and certifying agency requirements.
Ensures the highest standards of quality and professionalism in program services.
Attends intake and exit meetings as assigned.
Assists with scheduling program activities and Independent Options Facilitator.
Ensures all required client information is gathered and assessed to accurately determine eligibility and make recommendations for admission to the Supported Employment Program.
Prepares client files, including referrals, assessment information, and authorizations for services.
Tracks Supported Employment referrals and prepares reports regarding referral trends and unmet needs.
Assists with employment development reports and other program reports.
Provides clerical and administrative support, including answering phones, filing, data entry, and maintaining program records.
Provides orientation and training for Independent Options Facilitator.
Maintains client and program files in accordance with agency requirements.
Prepares and submits required reports in a timely manner and assists with billing.
Implements the EXCEED Injury and Illness Prevention Program.
Establishes and maintains effective working relationships with EXCEED programs, clients, funding agencies, referral sources, and community partners.
Assists with special events.
May represent EXCEED at meetings, trainings, and community events.
Drives regularly throughout the workday to conduct intake activities, attend meetings, travel between EXCEED locations, referral sources, and community partners as needed. Driving is an essential function of this position.
Performs other related duties as assigned.

QUALIFICATIONS, EDUCATION AND EXPERIENCE

Knowledge of intake, assessment, documentation, recordkeeping, and applicable regulatory requirements.
Ability to establish and maintain professional relationships with clients, referral sources, funding agencies, and community partners.
Strong organizational, communication, and customer service skills.
Professionalism and the ability to maintain confidentiality.
Proficiency with Microsoft Office and other computer-based systems.
Valid California Driver's License.
Acceptable driving record.
Current automobile insurance meeting California minimum requirements.
High school diploma or equivalent required.
Associate degree (A.A.) preferred.
Experience in human services, vocational rehabilitation, case management, intake, or a related field preferred.

PHYSICAL ABILITIES

Safely operate a motor vehicle and travel frequently throughout the service area to perform essential job duties.
Sit, stand, and walk for extended periods of time.
Sufficient manual dexterity to operate office equipment and prepare required reports.
Lift and carry up to 25 pounds.
Maintain balance and safely navigate office and community environments.
Stoop, kneel, crouch, and bend to retrieve stored materials or while demonstrating tasks during training or supervision.
Sufficient vision, hearing, and communication skills to safely perform the essential functions of the position, with or without reasonable accommodation.

Prioritize work, manage multiple tasks, and meet deadlines.

Perform basic mathematical calculations related to program documentation and reporting.
