Job description

Purpose:

NovumHealth is seeking a Facility Care Coordinator who works within a healthcare setting to provide comprehensive discharge and care coordination services to patients, ensuring their healthcare needs are effectively managed and coordinated. This role plays a vital part in enhancing outcomes, promoting and optimizing healthcare resources, and ensuring the client’s continuity of care needs are in place after an inpatient behavioral health crisis admission.

This position will be required to drive between different facility locations in the greater Pinellas and Pasco area. There may be intermittent travel to Hillsborough Area facilities if additional coverage is needed. Access to reliable transportation is required, which can safely get the employee from one location to another in under an hour. Mileage will be reimbursed at the IRS rate.

Key Responsibilities: (but not limited to)

Meet with members served in person while they are seeking care in a psychiatric inpatient setting. Assess for any discharge planning needs to help ensure a smooth transition back to the community.
Conduct member assessment and identify care plan needs that support safe return to the community upon discharge.
Ensure behavioral health 7 day-follow-up appointment and support services are scheduled.
Support member’s discharge plan and coordinate with facility social workers and discharge planners.
Coordinate the integrated care management needs of the member with the individual’s outpatient provider, hospital, PCP and others involved in his/her aftercare.
Perform telephone outreach to all members who could not be visited while inpatient and have recently been discharged / coordinate any needed support the member may need after discharge.
Outreach to members or their guardians to ensure 7-day follow-up appointment is completed.
Flexibility to attend multiple treatment settings same day to collect information and proactively monitor member's progress towards treatment goals/discharge.
Participate in Care Management/Treatment Team Meetings, in-person or virtually.
Uphold the organization’s ethics, code of conduct and customer services standards.

Knowledge, Skills and Abilities:

Knowledge of treatment plans
Good organizational and time management skills
Ability to write clear, concise documentation for record-keeping and billing purposes
Strong problem solving and communications skills (verbal and written)
Excellent crisis management skills
Ability to use electronic communications and internet resources.

Education, Licenses, Certifications and Experience:

Minimum of 1 year of experience in healthcare, care coordination, or a related field, preferably in a clinical or community healthcare setting.
Experience in working with diverse patient populations and addressing complex healthcare needs.

Education:

Bachelor's (Required) in psychology, social work, or related human services field
