Position Description: Community Outreach Liaison

Location: Belmont Elementary School

Position Type: Classified

Reports To: Principal

Qualifications: Bachelor in Social Work or a related field and relevant experience in a school setting.

Nature of Work
The Community Outreach Liaison promotes student success and well-being by strengthening partnerships
among the home, school, and community and by addressing barriers to attendance, learning, and achievement.
Each COL is responsible for providing the full range of liaison services described below at the school or schools
assigned by the district. The COL collaborates with students, families, school staff, district personnel, community
partners, and outside agencies to create a safe, supportive, and caring environment and to connect students and
families with appropriate interventions and resources.
Essential Duties and Responsibilities
? Relationships and engagement: Establish positive, respectful relationships with students, families, school
staff, and community partners. Serve as a consistent point of contact for families who need assistance
navigating school or community supports.
? Attendance and truancy support: Monitor and review attendance data; collaborate with administrators,
teachers, students, and parents or guardians to identify the root causes of absenteeism; support compliance
with North Carolina compulsory attendance requirements and district procedures; and implement
appropriate interventions and follow-up.
? Individualized intervention planning: Develop, implement, document, and monitor individualized plans
that address attendance, social, family, or other barriers affecting student success. Revise supports based on
student progress and changing needs.
? Student support and dropout prevention: Use available data, referrals, and individual assessment to
identify students at risk for chronic absenteeism, disengagement, or dropping out. Collaborate with
administrators, counselors, teachers, and families to provide targeted supports that promote engagement,
goal setting, successful transitions, and graduation.
? McKinney-Vento support: Help identify students who may be experiencing homelessness; promptly
communicate potential eligibility to the district McKinney-Vento liaison; assist families with referrals,
documentation, school access, transportation coordination, and other supports; and follow all applicable
federal, state, and district requirements.
? Domicile and enrollment support: Assist with domicile or residency concerns involving students assigned
to the COL’s school(s). Review available documentation, communicate with families, conduct home visits
when authorized and appropriate, document findings objectively, and submit information and
recommendations to the Director of Student Services or designee in accordance with district policy and state
requirements.

? Community services and referrals: Identify student and family needs and coordinate referrals to school-
based programs, community organizations, public agencies, faith-based partners, and other available

resources. Follow up, as appropriate, to determine whether services were accessed and whether additional
assistance is needed.
? Team participation: Participate in MTSS, attendance, student support, crisis response, and other
multidisciplinary meetings as assigned. Contribute relevant information, help identify systemic barriers, and
support coordinated plans for students and families.

? School and district coordination: Work collaboratively with other COLs, Student Success Advocates,
counselors, nurses, social workers, administrators, and Central Office staff. Participate in district meetings,
training, and coordinated initiatives to ensure services are consistent across schools.
? Records, reporting, and confidentiality: Maintain timely, accurate, objective, and confidential records of
contacts, referrals, interventions, home visits, services, outcomes, and required program documentation.
Prepare reports as requested and safeguard student and family information in accordance with FERPA,
district policy, confidentiality agreements, and other applicable requirements.
? Family and staff education: Provide information to students, families, and staff about attendance
expectations, available services, student protections, referral processes, and other topics related to the COL
role.

? Professional responsibilities: Maintain professional boundaries, use culturally responsive and family-
centered practices, complete required training, comply with Board policies and administrative procedures,

and perform other related duties as assigned.
Minimum Qualifications
? Education and experience: Bachelor’s degree in social work, education, counseling, or a related field
and/or relevant experience working with children, families, schools, or community agencies. Equivalent
combinations of education and experience may be considered in accordance with district requirements.
? Communication: Excellent verbal and written communication skills, including the ability to communicate
effectively with children, adults, families, school personnel, and community partners.
? Confidentiality and judgment: Demonstrated ability to maintain strict confidentiality, document
information objectively, exercise sound judgment, and handle sensitive situations professionally.
? Collaboration and independence: Demonstrated ability to organize and prioritize work, work
independently, collaborate as part of a multidisciplinary team, and follow through on multiple student and
family needs.
? Mobility: Ability to travel among assigned schools, homes, community agencies, and district locations as
required to perform job duties.

This position has access to confidential student, family, and personnel information. The employee is required to
sign a Confidentiality Agreement and comply with all applicable federal and state laws, Board policies, and
district procedures governing privacy and records.

Assignment of Responsibilities
Each Community Outreach Liaison is responsible for carrying out these duties at the school or schools assigned
by the district. Assignments may be adjusted based on enrollment, student needs, funding requirements,
staffing, or district priorities. COLs are expected to collaborate across schools when requested while maintaining
primary responsibility for their assigned school population.
