Position Overview
We are seeking an experienced Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) to provide in-person psychiatric services across our partner Skilled Nursing Facilities (SNFs) in the Roanoke, VA area.
This is a facility-based role focused on serving geriatric and long-term care residents. Providers will conduct psychiatric evaluations, medication management, follow-up visits, and collaborate closely with interdisciplinary care teams to improve patient outcomes and quality of life.
Responsibilities
- Conduct comprehensive psychiatric evaluations for SNF residents
- Diagnose and treat psychiatric and behavioral health conditions
- Provide psychotropic medication management and routine follow-up care
- Develop individualized treatment plans
- Collaborate with facility physicians, nursing staff, therapists, and social workers
- Ensure compliance with CMS, Medicare, and long-term care regulations
- Document patient encounters accurately within CharmHealth EHR
- Participate in interdisciplinary care planning and quality improvement initiatives
Qualifications Required
- Active PMHNP-BC license in Virginia
- DEA registration (or ability to obtain)
- Current Virginia APRN license
- Ability to travel between partner Skilled Nursing Facilities within the Roanoke area
- Strong clinical assessment and medication management skills
Preferred
- Previous experience in Skilled Nursing Facilities (SNFs), Long-Term Care (LTC), Assisted Living, or Geriatric Psychiatry
- Familiarity with Gradual Dose Reduction (GDR) guidelines and psychotropic medication management in accordance with CMS regulations
- Previous Medicare credentialing/enrollment
- Experience treating geriatric, dementia, and behavioral health populations
- Comfortable practicing independently while collaborating with facility care teams
