Full Job Description

SUMMARY:

NACC / Red Lake Wiinodewe Intensive Outpatient Program is a Native American, culturally rooted, harm reduction-based chemical health program that offers integrated health services. We are currently seeking an Intensive Outpatient Program Coordinator to join our team. An Intensive Outpatient Program (IOP) Coordinator is the critical link between patients, clinicians, and administrative systems, managing the logistical flow within treatment programming.

Ideal candidates will have experience working with Native communities, a strong understanding of recovery pathways, and a commitment to trauma-informed, culturally responsive care.

Job Duties:

Qualifications:

UMICAD or LADC
Have a valid driver’s license
High school diploma or higher
Experience working with underserved communities in the chemical health field.
Knowledge and understanding of ASAM criteria

Job Duties:

Program Coordination:

Respond to phone calls, messages, and general program inquiries and connect individuals with the appropriate staff or resources.
Support communication and coordination among clients, IOP staff, other NACC departments, and community partners.
Plan and coordinate client activities, program events, outreach activities, and community engagement.
Work with clinicians to provide follow-up care and connection to referred biopsychosocial services identified in client-relative treatment plans.
Support coordination tasks related to chemical health program expansion and strengthening external partnerships.

Client Access & Transportation:

Help navigate the practical steps required to access and participate in IOP services.
Coordinate transportation for clients, including arranging Ubers or other approved transportation to and from programming.
Identify barriers that may prevent clients from participating in services and work with staff to help address them.

Referrals, Intake & Scheduling:

Serve as a welcoming first point of contact for prospective client-relatives, families, referral partners, and others seeking programming information.
Receive incoming referrals and ensure consistent communication with the referent and potential client-relative to coordinate treatment admission and/or offer additional recovery care referrals.
Screen incoming referrals to determine if their ASAM level of care matches the programming level of care to participate in our Intensive Outpatient Program (IOP) and assess immediate recovery needs.
Schedule and complete intake paperwork with client-relatives who are admitting to IOP.
Review completed intake screeners with counselors and clinicians.
Provide orientation to new client-relatives enrolling in programming.
Maintain an organized system for tracking referrals, appointments, and follow-up needs.
Identify uninsured client-relatives and assist with gathering information needed to resolve insurance coverage issues.

PHYSICAL DEMANDS AND WORK ENVIRONMENT:

While performing the duties of this job, the noise level in the work environment is usually moderate.
