Job Description:

Job Summary:

The Transition Specialist provides information, counseling, and support to individuals and families about the transition process, assist participants in securing and mandating services that will assist them to leave an institutional setting and live in the community. Assistance includes information, coordination of services, short-term counseling, advocacy, and other supports to effectuate the transition. Transition Specialist also provides post-transition support and does outreach and education about the program and CDR services.

JOB DUTIES:

Coordinates visits to nursing homes and other institutions to provide residents, families, and discharge planners with objective information about available home and community-based and other potentially necessary services which may assist them to transition back to the community.
Works with the Disabled individual and nursing facility staff to develop a person-centered transition plan which ensures that the individual has access to housing, services, and other supports that allow the individual to transition into the community.
Coordinates the discharge-planning process, including funder organizations such as the Regional Resource Development Centers (RRDC), OPWDD, State Operations Office, and Regional Office's Front Door, and Managed Long-Term Care/ Managed Care Organizations.
Assist the participants in securing a community housing that meets their individual needs.
Connects participants with appropriate service providers who can assist them in implementing them in the appropriate portion of their service plan, review the outcomes of the participant's service plan, and advocate for the needs/ changes in these services in support of the participant's needs and desired outcomes.
Coordinates home and community services during the transition and ensure continuation of service for 90 days after the transition to community living.
Explain the peer mentor program to program participants and refer those interested in the program to it.
Attend appointments and meetings as necessary. Provide transportation to consumers as needed.
Contemporaneously and accurately document all services in compliance with CDR's standards and State DOH's regulations in both the CDR and NYAIL system according to each organization's policy.
Administers Quality of Life Surveys when requested on the timeline set forth by the NYAIL and/or the OPWDD system.
Participates in required trainings and maintain knowledge related to participant population. Transport participants to meetings, appointments, apartment tours, and lease signings.
Writes blog posts and news letter articles on program activities.
Other duties as assigned.

Qualifications:

A Bachelor's Degree in a human service-related field and two years of relevant experience working in a human service environment and/or with community-based long-term care services programs. An appropriate combination of skills, experience, and education can be accepted in lieu of a Bachelor's Degree.
Experience with case management, service coordination, or care management is desired.

Skills and Competencies:

Familiarity with various community-based services, HCBS Waiver, and all MLTC programs.
Comfort in working with people with physical, mental, and intellectual disabilities.
Demonstrated commitment to self-direction, person-centered planning and consumer choice.
Strong personal and written communication skills.
Excellent interpersonal, negotiation, and conflict resolution skills.
Strong organizational skills and attention to detail.
Impeccable time-management skills with a proven ability to meet deadlines.
