Full Job Description

Job Summary:

Community Habilitation services are individually designed for helping a person learn skills and get supports needed to pursue personal interests, integration, and independence.

Community Habilitation services are provided in the person's home and in the community. Habilitation staff is to assist individuals in achieving their goals, personal values and outcomes.

Community Habilitation Specialist Duties:

Provide services to persons served in their home and community according to their written Habilitation Plan and individual Safeguards.
Fluency in American Sign Language is strongly preferred
Document Habilitation services contemporaneously in accordance with the Agency, OPWDD, Medicaid and other requirements.
Provide input into the development and implementation of the Staff Action Plan in accordance with the person’s Life Plan as developed by each person receiving services, family, advocate, and Care Coordinator.
Monitor the safety and welfare of people served, take corrective actions as necessary and immediately notify supervisor of concerns.
Provide the required level of supervision as stated in the Staff Action and individual Safeguards.
Assist and support people served to build positive relationships in their community.
Provide needed support and encouragement to people served to advocate for their needs and rights.
Provide training and encouragement to people served to ensure good personal health care as needed.
Comply with all OPWDD regulations and CDR departmental policies and procedures.
Ability to be flexible and work varying hours depending on the need of the individual.
Other duties as assigned or required.

Qualifications:

Fluent in American Sign Language (ASL).
High School Diploma or General Equivalent Degree (GED).
One (1) year working with individuals with developmental disabilities.
Good oral and written communication skills.
Ability to read, write, speak, and understand English.
Ability to maintain confidentiality.
Ability to work independently and handle multiple tasks simultaneously.
Proficiency in the use of a personal computer and appropriate software and e-mail system.
Valid driver license and own vehicle required.
Possess a smart phone mobile device with data plan.

This position requires documenting services immediately after providing them. Are you comfortable using a personal computer/tablet for daily, web-based documentation?
Describe your experience supporting individuals with developmental disabilities to live more independently (e.g., cooking, shopping, hygiene, social skills).
Give an example of a time you worked independently in a community setting, handling multiple tasks or unexpected challenges.
If an individual you are supporting expresses a desire to do something that violates their safety plan (Safeguards), how would you handle the situation?
How would you encourage someone you support to advocate for their own rights or choices in a community setting?
As a Community Habilitation Specialist, you will be in family homes. How do you ensure confidentiality and respectful boundaries?
Are you familiar with, or comfortable learning, OPWDD documentation requirements.
You are assisting a person in the community and they become overwhelmed and refuse to follow their habilitation plan. What steps do you take?
A family member asks you to do a task that is not in the approved Habilitation Plan. How do you respond?
