Full Job Description

Role and Responsibilities

This position coordinates and ensures compliance to the Face-to-Face client visit with their primary care physician requirement. This role involves coordination and follow-up with clinical staff and physicians.

Essential Functions

Process Workflow in HCHB to produce Face-to-Face documents for physician signature, in accordance with the regulations.
Run reports and monitor compliance of returned forms and review for adequacy of documentation to meet the intent of federal regulations.
Coordinate with physicians/physician’s office staff and HCR clinical staff, as needed, to procure forms not completed.
Review returned documentation to ensure clinical components of the requirements are met.
Process documents through HCHB to allow billing to occur.
Other duties as assigned.

Education Requirements

Associate’s degree required.

Qualifications and Requirements

Current, valid LPN license from NYS Department of Education required.
This role will require critical thinking skills, strong organizational skills, and excellent communication skills.
There is also a need for clinical oversight/interpretation when reviewing the documentation.
Must possess basic computer skills – Email, Internet, Microsoft Office.

Work Environment

The Face-to-Face Coordinator LPN is primarily in an office setting and may be exposed to outdoor conditions.

The working conditions are classified as sedentary work:

Sedentary work - Exerting up to 10 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time.
