Compensation: $85,000/year (Midpoint) + Eligible for up to $1,000/month in performance bonuses
About the Role
Are you a Registered Nurse looking for an autonomous role that takes you out from behind a hospital desk and into the community? We are seeking a dedicated RN Medical Case Manager to support injured workers throughout their recovery and return-to-work journey across the Chicago metropolitan area.
Serving as a clinical advocate and care coordinator, you will work closely with injured workers, physicians, employers, and attorneys to ensure appropriate, cost-effective, and medically necessary treatment plans are implemented. This role seamlessly blends the flexibility of working from home with active, local field travel.

Key Responsibilities
- Care Coordination: Provide a mix of telephonic and in-person medical case management services; assess, plan, implement, and evaluate tailored treatment plans.
- Field Advocacy: Attend physician appointments, provider visits, and conduct initial patient assessments and home evaluations.
- Service Logistics: Coordinate vital recovery services, including durable medical equipment (DME), home health, and specialized nursing care.
- Stakeholder Collaboration: Monitor patient progress and communicate clear, objective updates to providers, employers, and insurance adjusters to support successful return-to-work outcomes.
- Documentation: Maintain highly accurate, timely clinical documentation and case records.
Qualifications & Requirements
Licensure & Education:
- Active, unrestricted Registered Nurse (RN) license in the state of Illinois.
- Graduate of an accredited School of Nursing (BSN preferred).
Experience:
- 1–3 years of clinical experience in Orthopedics, Neurology, Rehabilitation, Trauma, Occupational Health, or Acute Care.
- Preferred: Prior experience in Medical Case Management, Utilization Management, or Workers' Compensation.
Travel & Logistics:
- Ability to travel locally up to 60% of the time within the Chicago metropolitan area -- up to 3 days a week, including possibly up to 2 hours each way).
- Valid driver's license and reliable transportation suitable for year-round driving, including winter weather conditions.
Skills for Success:
- Autonomy: Proven ability to work independently, manage your own daily schedule, and prioritize a dynamic caseload.
- Tech Aptitude: Proficiency with Microsoft Office suite (Outlook, Word, and Excel) for remote documentation.
- Professional Growth: Willingness to obtain the Certified Case Manager (CCM) designation within 18 months of hire (fully funded by the company).
