We’re looking for a warm, dependable, and self-motivated Community Outreach Representative to help us grow our presence throughout the community and connect families with quality home care services.

We’ve already secured several Lunch & Learn presentations with healthcare partners, giving you a great starting point as you begin building relationships in the community. You’ll also play a key role in expanding our outreach by identifying new partnership opportunities, coordinating future Lunch & Learns, participating in community events and networking opportunities, and helping increase awareness of Guardian Healthcare throughout York County.

You focus on building relationships—we’ll support you behind the scenes.

Our office team provides administrative support, scheduling assistance, marketing materials, and leadership guidance so you can spend more time building relationships in the community and less time behind a desk.

What You’ll Do

As our Community Outreach Representative, you’ll represent Guardian Healthcare by building strong relationships with healthcare professionals, community organizations, and local partners.

Your outreach may include visits to:

Hospitals
Physician offices
Rehabilitation facilities
Assisted living and memory care communities
Hospice providers
Senior centers
Community organizations
Other referral partners

Responsibilities Include:

Build and maintain professional relationships with healthcare providers, community organizations, and referral partners.
Conduct both follow-up visits and outreach to new referral sources.
Identify, coordinate, and participate in community events, networking opportunities, and educational outreach activities.
Help schedule, organize, and present Lunch & Learn sessions with healthcare facilities and community organizations.
Represent Guardian Healthcare at local events and professional networking opportunities.
Educate healthcare professionals, community partners, and families about our home care services.
Share updates from your outreach activities with the office team to help keep outreach organized and support future planning.
Work closely with leadership to identify new opportunities for community partnerships and agency growth.

Requirements

Valid driver’s license
Reliable transportation
Comfortable traveling throughout York County and surrounding areas

Preferred:

Experience in community outreach, healthcare, business development, relationship management, or outside sales.
Experience working with healthcare providers, senior services, or community organizations is a plus.
