Full Job Description

Funded by a US Dept of Education grant awarded to Lancaster County Partners for Youth, the Lancaster Promise Neighborhood Family Services Coordinator duties and responsibilities will include, but are not limited to, serving as part of the LPN team and specifically being responsible for supporting and advocating for families impacted by LPN services as well as the following.

ESSENTIAL FUNCTIONS OF THE JOB

1. Client Support: The coordinator will provide comprehensive support to families, addressing their unique needs and connecting them with appropriate services.

2. Supervision: The coordinator will supervise a staff of Academic Case Managers (Success Coaches) at LPN’s schools of focus.

3. Assessment: The coordinator will conduct thorough assessments of family situations, identifying challenges and strengths to develop tailored service plans.

4. Advocacy: The coordinator will advocate on behalf of families to help to ensure they have access to necessary resources, services, and support systems.

5. Coordination: The coordinator will collaborate with community agencies, schools, and healthcare providers to coordinate and enhance services for families.

6. Education: The coordinator will offer educational programs on topics relevant to family well-being, fostering a supportive and informed community.

7. Crisis Intervention: The coordinator will respond to crises within families, providing immediate support and facilitating appropriate interventions.

8. Documentation: The coordinator will maintain accurate and confidential records of client interactions, assessments, and service plans.

9. Community Outreach: The coordinator will engage in outreach activities to raise awareness of available services and build partnerships with other organizations.

10. Empowerment: The coordinator will empower families to develop the skills and resources needed to navigate challenges independently.

11. Reporting: The coordinator will generate regular reports on program outcomes, highlighting successes and areas for improvement.

QUALIFICATIONS

Bachelor's degree in social work, psychology, or related field; Master's degree preferred.
Proven experience in family services or a related field.
Strong interpersonal, communication, and organizational skills.
Knowledge of community resources and support services.
Ability to work collaboratively with diverse populations.
Sensitivity to cultural differences and commitment to diversity and inclusion.

· Tasks require oral and written communication skills as well as excellent analytical and project management skills. Strong organizational skills and ability to manage multiple tasks.

· Commitment to the goals, values, and approach of Promise Neighborhood.

· Knowledge and familiarity of the community, comfort in working in various neighborhood settings with diverse populations.

· Willingness to travel and work evenings and weekends.

ACCEPTABLE EXPERIENCE AND TRAINING

Graduation from an accredited college or university with a degree in in social work, psychology, or related field, with a preferred equivalent combination of five (5) years of minimum experience and training which provides the required knowledge, skills, and abilities.

From time to time the incumbent may be assigned special duties by a superior or, on a temporary basis, may be asked to assist other employees occupying a different position. Overtime work will be required on occasion. ADA REQUIREMENTS

Physical Requirements: Tasks involve frequent walking, standing, and carrying (10-20 lbs.); manual dexterity in the use of fingers, limbs or body in the operation of shop and office equipment; may involve extended periods of sitting at keyboard.
