Full Job Description

First Care Medical Clinic is seeking a dedicated and clinically rigorous LCSW, LCSWA, LCMHC, or LCMHCA to serve as a Behavioral Health Therapist. This position is focused on the delivery of high-quality, evidence-based individual and group therapy in a medical office setting and group home setting.

FCMC Mental Health Therapist – Job Requirements and Expectations for Employment:

· This is an in-person position.

· Mental Health Therapists work 5 days per week at First Care Medical Clinic.

· Mental Health Therapists are not permitted to maintain a private practice or solicit First Care Medical Clinic for their own practice.

· The standard shift begins promptly at 9:00 AM ET. Arrival and clock-in at 9:00 AM ET constitutes on-time arrival. The shift is scheduled to end at 6:00 PM ET.

· Weekly schedule for the Mental Health Therapist includes scheduled appointments with behavioral health patients.

· The use of Sunoh.AI is required for all visits. Sunoh.AI is the AI transcription tool within eClinicalWorks, the electronic medical record used by First Care Medical Clinic.

· Weekly schedule for the Mental Health Therapist includes time to meet with a consulting psychiatrist MD and/or Psychiatric Mental Health Nurse Practitioner (PMHNP).

· Mental Health Therapists address real-time referrals from primary care providers as part of First Care Medical Clinic’s integrated care model.

· Schedules are determined based on clinic needs; FCMC is unable to accommodate requests for customized schedules (e.g., guaranteed days off, specific shift lengths).

· Chronic absenteeism or repeated tardiness will result in disciplinary action, up to and including termination.

· Violation of First Care Medical Clinic’s Code of Conduct or policies is grounds for immediate disciplinary action, up to and including termination.

· Failure to achieve or maintain the required level of competence for all Behavioral Health providers is grounds for termination.

· Patient discharge: Mental Health Therapists are required to fill out a patient discharge form, and have medical director approve of the form, before a patient is officially discharged. Behavioral Health providers cannot discharge patients without medal director signing off on discharge form.

· Daily: If a patient misses a visit, the Mental Health Therapist is required to complete a chart review, and document the no show visit. The no show documentation must include medium of engagement, planned objective for visit, attempts to reach out to patient, and the date of next visit.

· Daily: Mental Health Therapists are required to keep track of no-shows in eClinicalWorks. If patient misses a visit, Mental Health Therapist are required to call or text patient at end of day to re-engage with patient.

· Weekly: Mental Health Therapist are required to bring a patients to review with psychiatric consultant in every weekly session.

· Weekly: Mental Health Therapist will meet with medical director after psychiatric consult sessions to discuss treatment recommendations by consulting psychiatrist.

· Weekly: Mental Health Therapist will meet with the medical director and operations lead weekly to discuss patient case load and continuation of care.

· Weekly: Mental Health Therapist will have charts audited to provide coaching and guidance on documentation.

· Daily: Mental Health Therapists are required to use timer for all activities tied to a specific patient, including, patient visits, psych consults, PCP consults, chart reviews, and care coordination.

o Outreach and engagement of patients

o Initial assessment, including administration of validated scales and resulting in a treatment plan

o Entering patients into a registry and tracking patient follow-up and progress

o Participation in weekly caseload review with Psychiatric Consultant

o Provision of brief interventions using evidence-based treatments such as Behavioral Activation, Problem Solving Treatment, and other focused treatment activities

o Monitoring patient outcomes using validated rating scales

o Ongoing review by Psychiatric Consultant and treatment modifications based on recommendations

o Relapse prevention planning and preparation for discharge from active treatment

Responsibilities

· Direct Clinical Practice: Deliver individual, family, and group psychotherapy sessions to a diverse patient population presenting with a wide range of psychological and emotional challenges.

· Evidence-Based Treatment: Utilize proven therapeutic modalities—such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), Acceptance and Commitment Therapy (ACT), and Trauma-Informed Care—tailored to the specific needs of the patient.

· Treatment Planning: Develop and implement comprehensive, goal-oriented treatment plans. Regularly review progress and adjust therapeutic strategies to ensure efficacy and patient growth.

· Diagnostic Integrity: Conduct thorough diagnostic assessments, utilizing the DSM-5-TR to provide accurate clinical diagnoses and formulate treatment rationales.

· Sustain Therapeutic Alliance: Build and maintain strong, professional therapeutic relationships characterized by empathy, boundaries, and a commitment to patient autonomy.

· Process Consistency: Adhere to standardized clinical protocols for intake, treatment planning, and discharge to ensure a uniform level of care across all locations.

· Documentation Excellence: Maintain highly detailed, session-by-session clinical notes in the EHR that clearly articulate therapeutic interventions and patient responses, meeting all state regulatory and insurance requirements.

· Peer Consultation: Participate in regular clinical supervision and peer review sessions to ensure the highest standards of care and continued professional growth.
