Full Job Description

We are currently seeking experienced Hospice Registered Nurses in Dallas, Texas and surrounding areas to deliver caring with compassion, commitment, and professionalism to the local community. As a Registered Nurse, you will:

Work one-on-one with patients in a home environment
Computerized documentation
Provide Hospice care to the patient as per physician’s orders and by instruction to LVN’s, Home Health Aides, and other qualified staff.
Perform Responsibility for Patient Admission, Plan of Care, Re-certification, and Discharge.
Exercise strong decision making and judgment skills.
Collaborate and coordinate the care with the interdisciplinary team.
Work with an interdisciplinary team in weekly IDT meetings.

Responsibilities:

Assess patient/family for physical, psychological, social, and spiritual needs and to establish the plan of care, implement interventions.
Effective communication with attending physician, Medical Director, IDG to coordinate care and use of resources for the patient/family.
Collaborate with the interdisciplinary team to develop and implement individualized care plans
Provide quality of care by scheduling patient visits in accordance with plan of care.
Participates in IDG Meetings, patient / family meetings in coordination with IDG and Medical Director
Educate patients and their families on disease management, end-of-life care, and available resources
Promotes the Agency 's philosophy and administrative policies.

Qualifications:

Must be licensed in the State of Texas.
Diploma, Associate, of Baccalaureate degree in nursing
One year Hospice or Home Health care experience preferred.
Must be CPR certified.
Must work well independently, enjoy problem solving and understand the importance of accurate documentation.
Must have critical thinking skills.
Must have exceptional interpersonal skills.
Valid Driver’s license and proof of automobile insurance.
