Full Job Description

Position: Licensed Practical Nurse (LPN) / Medical Coordinator
Schedule: Monday–Friday, 8:00 AM – 4:00 PM (Hours may vary due to medical appointments, emergencies, or other job-related responsibilities.)

Position Summary

The LPN/Medical Coordinator plays a vital role in ensuring the health, safety, and well-being of individuals served by the Robert L. Kyle Center. This position is responsible for coordinating medical care, maintaining accurate medical records, overseeing medication management, and providing direct support services when needed. The LPN/Medical Coordinator works closely with physicians, pharmacies, families, guardians, and program staff to ensure individuals receive quality healthcare while promoting independence and community inclusion.

Essential Duties and Responsibilities Medical Coordination

Schedule and coordinate all medical, dental, vision, and specialty appointments.
Transport individuals to and from medical appointments as needed.
Communicate with physicians, nurses, pharmacies, guardians, and other healthcare providers.
Ensure physician orders are received, reviewed, and implemented accurately.
Maintain current medical records and documentation for each individual.
Complete annual medical paperwork and ensure all required health assessments are current.
Monitor follow-up appointments, lab work, and recommended treatments.
Assist with medical emergencies and communicate concerns to the appropriate supervisor.

Medication Management

Order prescription medications and medical supplies.
Receive, check, and verify all medications upon delivery.
Ensure Medication Administration Records (MARs) are accurate and current.
Audit medications and MARs regularly for accuracy and compliance.
Coordinate medication changes and ensure staff are informed of updates.

Direct Support Professional (DSP) Responsibilities

The LPN/Medical Coordinator is also expected to work as a Direct Support Professional and provide direct care to individuals served. Duties include:

Assist individuals with daily living skills and personal care as needed.
Support individuals with housekeeping, laundry, meal preparation, and maintaining a clean home.
Assist individuals with grocery shopping and other community errands.
Encourage participation in community activities, appointments, and recreational outings.
Promote independence, dignity, and person-centered supports.
Complete required daily documentation accurately and on time.
Follow all agency policies, state regulations, HIPAA requirements, and safety procedures.

Qualifications

Current Missouri Licensed Practical Nurse (LPN) license in good standing.
Valid Missouri driver's license (Class E preferred or ability to obtain if required).
Acceptable driving record and ability to transport individuals safely.
CPR/First Aid certification (or ability to obtain).
Ability to pass state and federal background checks.
Strong organizational, communication, and time-management skills.
Ability to maintain confidentiality and comply with HIPAA regulations.
Experience working with individuals with developmental disabilities is preferred but not required.

Physical Requirements

Ability to lift, transfer, push, or pull up to 50 pounds.
Ability to assist individuals with mobility and personal care.
Frequent walking, standing, bending, and reaching.
Ability to safely operate agency vehicles.
