Full Job Description

Healing Hope Community, Inc. (HHCI) is one of Maryland’s newest non-profit mental health organizations. HHCI prides itself in being a clinician-led organization committed to strengthening and enhancing the lives of individuals experiencing mental health challenges in the community. We aspire to be a leading non-profit mental health organization that prioritizes the needs of our clients and personnel, which ultimately places everyone in a position to be successful and thrive in their environments.

HHCI is in search of a Peer Specialist (PS) for its Assertive Community Treatment (ACT) Team. This position is currently field-based, and will require the PS to report to the office (located in Baltimore, MD) as needed.

Position Summary:

The ACT team PS provides ACT services to a mutually shared caseload of ACT clients within HHCI’s treatment and recovery services division. The PS is responsible for utilizing evidence-based practices to assist their clients with meeting their treatment and recovery goals, and operating within the scope of their profession. This includes peer recovery support, wellness management, psychoeducation, motivational interviewing, crisis support, and trauma-informed care.

Responsibilities:

Provide ACT services to each admitted client of the ACT team at least monthly
Conduct mental health assessments as needed and directed by the CD
Coordinate referrals for additional support for admitted clients
Accompany clients to appointments, if needed
Enter, sign, and submit all electronic clinical documentation to the CD within 24 hours of the encounter with the client, including progress notes and assessments
Conduct intake screenings for prospective clients
Prepare for, attend, and participate in daily team meetings
Engage in person-centered treatment planning with clients
Conduct requested self-audits of charts for assigned clients, as well as unplanned chart audits
Collaborate with consented external providers and internal personnel regarding the treatment goals of mutually shared clients
Complete required trainings within established timeframes
Rotate the 24-hour on-call ACT phone with other members of the team
Comply with all relevant standards and regulations of HHCI, the state of Maryland’s Behavioral Health Administration (BHA), The Joint Commission, HIPAA
Perform other duties as requested by the CD

Required Qualifications:

High School Diploma
Certified in the state of Maryland as a Certified Peer Recovery Specialist (CPRS)
A minimum of 1 year of providing peer recovery support/counseling to the mental/behavioral health population
Proficiency in Microsoft Suite (Word, Excel, Outlook, Power Point, Access), electronic health records, and the use of computers/laptops

Preferred Qualifications:

Strong preference for an associate's or bachelor’s degree in Social Work (BSW), Psychology, Human Services, or a related field
Strong preference for experience working with a multi-disciplinary mobile team

Pay: $27.00 - $32.00 per hour
